Dizziness

Doctor Hsu, I have been experiencing dizziness and a sensation of spinning for the past 2 to 3 days.
I also have episodes of tinnitus more than five times a day, especially when lying down to sleep.
What could be the cause of this?

Shui Er, 30~39 year old female. Ask Date: 2006/10/10

Dr. Xu Jiazhen reply Otolaryngology

Dizziness, or the sensation of spinning (vertigo), can be a distressing experience, and understanding its causes is crucial for effective management. From an ENT (Ear, Nose, and Throat) perspective, dizziness can arise from various conditions, primarily related to the vestibular system, which is responsible for balance.

One common cause of dizziness is Benign Paroxysmal Positional Vertigo (BPPV). This occurs when tiny calcium carbonate crystals in the inner ear become dislodged and move into the semicircular canals, leading to brief episodes of vertigo, especially when changing head positions. If your dizziness is triggered by specific movements, this could be a likely cause.

Another potential cause is Vestibular Neuritis, an inflammation of the vestibular nerve, often following a viral infection. This condition can lead to prolonged episodes of vertigo, accompanied by nausea and balance issues. Unlike BPPV, the dizziness from vestibular neuritis can last for days or weeks.

Meniere's disease is another condition that can cause episodes of dizziness, often accompanied by tinnitus (ringing in the ears) and hearing loss. This condition is related to fluid buildup in the inner ear and can lead to more severe episodes of vertigo.

Labyrinthitis, an infection or inflammation of the inner ear structures, can also cause dizziness. This condition may follow a respiratory infection and can present with both hearing loss and vertigo.

In addition to these vestibular disorders, dizziness can also be associated with other non-ENT related issues, such as anxiety, dehydration, or low blood pressure. It’s essential to consider your overall health, including any recent changes in medication, hydration levels, or stress, as these can contribute to feelings of dizziness.

To address your symptoms, it is advisable to:
1. Consult an ENT Specialist: A thorough evaluation by an ENT specialist is crucial. They may perform a physical examination, including tests to assess your balance and hearing, and may recommend imaging studies if necessary.

2. Vestibular Rehabilitation Therapy (VRT): If your dizziness is diagnosed as BPPV or another vestibular disorder, VRT can help retrain your brain to compensate for the inner ear issues.

3. Medication: Depending on the diagnosis, medications may be prescribed to alleviate symptoms. For instance, antihistamines can help with vertigo, while anti-nausea medications can assist with associated nausea.

4. Lifestyle Modifications: Staying hydrated, avoiding sudden movements, and managing stress can help reduce the frequency and severity of dizziness episodes.

5. Follow-up: Regular follow-up appointments may be necessary to monitor your condition and adjust treatment as needed.

In summary, dizziness can stem from various causes, particularly related to the vestibular system. A comprehensive evaluation by an ENT specialist is essential to determine the underlying cause and develop an appropriate treatment plan. If you experience persistent or severe dizziness, it is crucial to seek medical attention promptly to rule out any serious conditions.
