Penile issues
Hello doctor, for the past two months, I have been feeling something flowing inside my penis.
It doesn't hurt and there is no swelling.
It has been happening intermittently for two months.
I also notice a slight bifurcation in my urine stream and some bubbles.
Last month, I had a circumcision, and the recovery has been okay.
Previously, I had a small wart at the urethral opening, which was burned off.
Could it be that there are warts inside the urethra causing these strange sensations in my penis, or could it be due to urethritis? I'm really worried because this has been going on for over two months.
It feels tingly, but it seems like something is flowing inside my penis.
Sometimes, I feel like the blood is rushing quickly near the penile area.
What could be wrong?

Concerned individual: Hello, your description is quite unique.
I would like to confirm one point: do you feel the sensation of fluid movement within the penis as a feeling in the urethra or within the penile tissue itself? If it is in the urethra, it is generally associated with infections or conditions like phimosis or cauterization of warts, and typically, such procedures do not result in these types of sequelae.
If necessary, you may want to have a urinalysis or other tests done for a preliminary diagnosis.

More Info
Dear Patient,
Thank you for reaching out with your concerns regarding the symptoms you are experiencing. It’s understandable to feel anxious when dealing with unusual sensations in such a sensitive area. Based on your description, there are several potential causes for the symptoms you are experiencing, including urethritis, possible residual effects from your recent circumcision, or even the presence of a sexually transmitted infection (STI) such as HPV, which can cause genital warts.
1. Urethritis: This is an inflammation of the urethra, which can be caused by infections, including STIs or non-infectious factors. Symptoms may include a burning sensation during urination, discharge, and sometimes a feeling of pressure or unusual sensations in the penis. The fact that you mentioned a "flowing" sensation could be related to inflammation or irritation in the urethra.
2. Post-Circumcision Effects: After a circumcision, it’s common to experience some changes in sensation as the area heals. The healing process can sometimes lead to unusual sensations, including tingling or a feeling of fullness. If you have recently undergone this procedure, it’s important to follow up with your healthcare provider to ensure that the healing process is proceeding normally.
3. Genital Warts: You mentioned having a small wart-like growth that was treated. HPV can cause warts in the genital area, and while the visible warts may have been removed, the virus can remain in the body and potentially cause further issues. If there are new growths or changes in sensation, it’s crucial to have these evaluated by a healthcare professional.
4. Possible Infection: The presence of bubbles in your urine and the sensation of something moving could indicate an infection, such as a urinary tract infection (UTI) or a sexually transmitted infection. These infections can cause changes in urination patterns, including splitting of the urine stream and unusual sensations.
Given that your symptoms have persisted for over two months, it is essential to seek medical attention. A healthcare provider can perform a thorough examination, which may include a physical examination, urine tests, and possibly imaging studies to determine the underlying cause of your symptoms. They may also consider testing for STIs, especially if there is a history of exposure.
In the meantime, it’s advisable to avoid any irritants, such as harsh soaps or lotions in the genital area, and to maintain good hygiene. Staying hydrated can also help with urinary symptoms.
Please do not hesitate to reach out to a urologist or your primary care physician for a comprehensive evaluation. Early diagnosis and treatment are key to managing any potential issues effectively. Your health and peace of mind are important, and addressing these symptoms promptly will help ensure the best outcome.
