Enlarged facial pores?

I have been experiencing two significant issues with my face over the past six months:
1.
Initially, I only had enlarged pores on either side of my nose, but in the past few months, the pores on my entire cheeks have become visibly enlarged and are filled with something resembling white cotton-like material, with some ends being thin and long (similar to fine hair).
What started as just my cheeks has now gradually spread to my temples as well.
I have tried using a comedone extractor to remove them, but there are too many to clear out completely, and the texture of what I extracted does not resemble typical comedones.
A dermatologist mentioned that it is a blockage and advised me to wash my face four times a day and apply a medicated acne lotion, but there has been no improvement after two weeks.

Additionally, near the hairline or around my ears on both cheeks, I develop bumps or rashes that resemble mosquito bites every couple of days.
They are very itchy, and the more I scratch, the more they appear, but they disappear after a few hours, although some leave scars.
One doctor suggested it might be urticaria and prescribed a topical corticosteroid, while another suggested it is also a blockage and recommended more frequent face washing and the use of medicated acne lotion, but again, there has been no improvement.
