Fecal impaction
Doctor, I have been experiencing issues with irregular bowel movements and constipation for some time now.
I have seen a doctor and am taking Western medication.
However, I notice that my stomach remains distended even after using the restroom.
In the past, my stomach would feel smaller after a bowel movement.
What could be the reason for this? Is it because there is still stool retained, or has my stomach become larger?

Hello Ms.
Sa: If you have daily bowel movements, then there is no issue with retained stool (though you may experience bloating).
If the swelling continues to increase, you may need to visit a gastroenterologist for further examination and medication.

Constipation is a common gastrointestinal issue that can lead to a variety of uncomfortable symptoms, including bloating and a feeling of fullness in the abdomen, even after a bowel movement. The experience you describe—having a bloated belly despite having just used the restroom—can be attributed to several factors.
Firstly, it’s important to understand that constipation is not just about the frequency of bowel movements; it also involves the consistency of the stool and the ease with which it can be passed. When you are constipated, stool can become hard and dry, making it difficult to eliminate. Even after a bowel movement, if there is still stool remaining in the intestines, it can contribute to a feeling of fullness or bloating. This residual stool is often referred to as "stool retention" or "fecal impaction," which can occur when the body is unable to fully evacuate the bowels.
Another reason for persistent bloating after a bowel movement could be related to the gut's motility. If your intestines are not moving efficiently, food and gas can accumulate, leading to a sensation of fullness. This can be exacerbated by dietary choices, such as consuming high amounts of fiber without adequate hydration, which can lead to gas production and bloating.
Additionally, the sensation of bloating can also be influenced by the gut microbiome. An imbalance in gut bacteria can lead to increased gas production, which can cause bloating even after a bowel movement. This is particularly relevant if you have made recent changes to your diet or if you are taking medications that affect gut flora.
Stress and anxiety can also play a significant role in gastrointestinal symptoms. The gut-brain connection is well-documented, and emotional stress can lead to changes in gut motility and increased sensitivity, which may contribute to feelings of bloating and discomfort.
If you are currently taking medication for constipation, it’s worth discussing with your healthcare provider whether the medication is effective for you. Some medications can take time to work, and others may not address the underlying causes of your symptoms. It may also be beneficial to explore dietary changes, such as increasing fiber intake gradually while ensuring adequate hydration, as well as incorporating probiotics to support gut health.
In summary, the feeling of bloating after a bowel movement can be due to residual stool, inefficient gut motility, dietary factors, an imbalance in gut bacteria, or even stress. If these symptoms persist, it is advisable to follow up with your healthcare provider to explore further diagnostic options and treatment strategies tailored to your specific situation. Regular follow-ups can help ensure that any underlying issues are addressed, and you can achieve better digestive health.
