Hemorrhage after ejaculation?

Hello doctor: A few days ago, I noticed what seemed like a abrasion at the urethral opening, but there was no bleeding, and it healed the next day.
However, today I experienced some bleeding after ejaculation, although the amount is not significant.
This has never happened to me before.
I have checked and it seems there is a slight abrasion on the inner side of the urethra.
Why could this be happening? Could it be due to excessive force, or is it a sign of some underlying condition that could lead to this?

Benben, 10~19 year old female. Ask Date: 2006/11/07

Dr. Pan Zhenqin reply Urology

Excessive force during sexual intercourse can potentially cause minor injuries to the urethral mucosa, which usually are not serious.
If this occurs repeatedly or if there are concerns, it is advisable to consult a urologist for a thorough examination.

Post-ejaculation bleeding can be concerning and may arise from various causes. Based on your description, it seems that you experienced some irritation or abrasion at the urethral opening, which could be a significant factor in the bleeding you observed after ejaculation. Here’s a detailed breakdown of potential causes and considerations regarding your situation.

Possible Causes of Post-Ejaculation Bleeding
1. Urethral Injury or Irritation: If you have experienced any form of trauma or excessive friction during sexual activity, it could lead to minor abrasions or irritation of the urethra. This is particularly common if there was vigorous activity or if lubrication was insufficient. The presence of blood after ejaculation could indicate that the urethra was slightly damaged, leading to bleeding.

2. Infections: Urinary tract infections (UTIs) or sexually transmitted infections (STIs) can cause inflammation and irritation of the urethra, which might result in bleeding. Symptoms of infection can include burning during urination, increased frequency of urination, or unusual discharge. If you suspect an infection, it is advisable to seek medical attention.

3. Prostatitis: Inflammation of the prostate gland, known as prostatitis, can also lead to bleeding during or after ejaculation. This condition may be accompanied by other symptoms such as pelvic pain, difficulty urinating, or discomfort in the genital area.

4. Benign Prostatic Hyperplasia (BPH): Although more common in older men, BPH can lead to urinary symptoms and sometimes bleeding. This condition involves enlargement of the prostate, which can affect urinary flow and lead to irritation.

5. Other Medical Conditions: Conditions such as hemophilia or other bleeding disorders can also lead to unusual bleeding patterns. If you have a history of bleeding disorders or if you are on anticoagulant medications, this could be a contributing factor.

Recommendations
- Monitor Symptoms: Keep an eye on any additional symptoms that may develop, such as pain during urination, fever, or changes in urinary habits. If bleeding persists or worsens, it is essential to seek medical attention.

- Avoid Irritation: Until you have a clearer understanding of the cause, consider avoiding activities that may further irritate the area, such as vigorous sexual activity or using harsh soaps or products around the genital area.

- Seek Medical Evaluation: If the bleeding continues or if you experience any concerning symptoms, it is crucial to consult a healthcare professional. They may perform a physical examination, conduct urine tests, or recommend further investigations to determine the underlying cause of the bleeding.

- Consider Sexual Health: If you are sexually active, it’s essential to practice safe sex to reduce the risk of STIs, which can also lead to complications such as bleeding.

In conclusion, while occasional minor bleeding after ejaculation can occur due to irritation or minor injury, it is important to remain vigilant and seek medical advice if symptoms persist or worsen. Understanding the underlying cause is crucial for appropriate management and to ensure your overall health and well-being.
