Erection Issues: Causes of Reduced Hardness and Size - Urology

Insufficient rigidity after erection?

Hello doctor, it seems that my erection used to be around 15 centimeters, but now it's only about 13 centimeters.
The most concerning part is that even with 13 centimeters, it doesn't feel very firm, and the angle cannot exceed 90 degrees.
I feel quite discouraged.
I also masturbate quite often, about 3 to 4 times a week.
The base of my penis is noticeably less firm.
Could frequent masturbation cause any injury to the white membrane or something similar?

Amin, 20~29 year old female. Ask Date: 2006/11/11

Dr. Xu Weikai reply Urology

A-min: In Western medicine, there is no notion that masturbation affects bodily functions.
On the contrary, it is believed that the ability to achieve an erection and ejaculate typically indicates a certain level of health; in other words, if the body is not healthy, one may struggle to achieve an erection.
The size of the penis is primarily related to the level of arousal and does not seem to have a direct correlation with the frequency of masturbation.
The most likely reason for any perceived changes is that long-term masturbation may lead to a habituation effect, resulting in a diminished capacity for intense arousal over time, which can naturally affect the strength of erections.

Aside from the aforementioned issues, a commonly discussed downside of masturbation is the problem of premature ejaculation.
Due to the habit of masturbation, individuals may not consider their partner's needs, leading to increasingly rapid actions.
In actual sexual intercourse, especially for those who are inexperienced or just starting out, some individuals may indeed experience premature ejaculation.
However, with learning and adaptation, this typically normalizes over time.

More Info

Erection issues, particularly those involving reduced hardness and size, can be a source of concern for many men. The symptoms you describe, including a decrease in erection firmness and a change in size, can stem from various physical and psychological factors. Understanding these causes is crucial for addressing the issue effectively.

Physical Causes
1. Circulatory Issues: Erection is primarily a vascular event. Blood flow to the penis is essential for achieving and maintaining an erection. Conditions such as atherosclerosis (hardening of the arteries), hypertension, or diabetes can impair blood flow, leading to difficulties in achieving a firm erection.

2. Hormonal Factors: Testosterone plays a significant role in sexual function. Low testosterone levels can lead to reduced libido and erectile dysfunction. Hormonal imbalances can be assessed through blood tests, and if low testosterone is identified, hormone replacement therapy may be an option.

3. Nerve Damage: Conditions such as diabetes or pelvic surgeries can lead to nerve damage, affecting the signals necessary for an erection. Neuropathy can result in diminished sensitivity and erectile difficulties.

4. Psychological Factors: Stress, anxiety, depression, and relationship issues can significantly impact sexual performance. Psychological factors can lead to performance anxiety, which may exacerbate erectile dysfunction.

5. Lifestyle Choices: Factors such as smoking, excessive alcohol consumption, and lack of physical activity can contribute to erectile issues. A healthy lifestyle can improve overall vascular health and, consequently, erectile function.

Impact of Masturbation
Regarding your concern about frequent masturbation, it is generally considered a normal sexual activity and is not typically associated with long-term damage to the penis. However, excessive or aggressive masturbation can lead to temporary sensitivity or soreness. If you notice any pain or discomfort, it may be beneficial to reduce the frequency or intensity of your activity.

Recommendations
1. Consult a Healthcare Provider: It is essential to discuss your symptoms with a healthcare professional. They can perform a thorough evaluation, including physical examinations and blood tests, to identify any underlying conditions contributing to your erectile issues.

2. Lifestyle Modifications: Consider adopting healthier lifestyle habits. Regular exercise, a balanced diet, and avoiding smoking and excessive alcohol can improve overall health and potentially enhance erectile function.

3. Psychological Support: If you suspect that psychological factors are at play, consider speaking with a therapist or counselor specializing in sexual health. Cognitive-behavioral therapy (CBT) can be particularly effective in addressing performance anxiety and other psychological barriers.

4. Medication: If appropriate, your doctor may prescribe medications such as phosphodiesterase type 5 inhibitors (e.g., Viagra, Cialis) to help improve erectile function. These medications work by increasing blood flow to the penis during sexual arousal.

5. Regular Check-ups: Regular health check-ups can help monitor and manage any chronic conditions that may affect erectile function, such as diabetes or cardiovascular disease.

Conclusion
Erection issues can be multifaceted, involving a combination of physical and psychological factors. It is crucial to approach the problem holistically, considering both medical and lifestyle aspects. Seeking professional advice is the best course of action to identify the underlying causes and develop an effective treatment plan tailored to your needs. Remember, you are not alone in this, and many men experience similar issues at various points in their lives.
