Urinary incontinence
Hello doctor, I am currently 17 years old and almost 18.
When I urinate, the stream splits, making it difficult to aim, and I also experience slight leakage after urination.
I previously consulted a urologist who advised me to retract my foreskin while urinating, which I have been doing, but the situation doesn't seem to improve.
Could this be related to holding in my urine? Sometimes I do hold it in, or could it be related to masturbation? I masturbate about once every 3 to 4 days.
I would appreciate your guidance, thank you.

Dr. Lai Ronghao reply Urology
Hello, if you experience dribbling after urination, it is likely due to residual urine in the urethra.
You can try shaking gently after urination and lifting the scrotum to help any remaining urine flow out completely, which may improve the situation.
If you have any further concerns, please visit a urology clinic for further evaluation and treatment.
Thank you!

More Info
Hello! It's great that you're reaching out to discuss your urinary issues, as these can be quite concerning, especially at your age. Based on your description, it sounds like you're experiencing two primary symptoms: a split stream during urination and some post-void leakage. Let's break down these issues and explore potential causes and solutions.
Split Stream During Urination
A split stream can occur for several reasons, and it's not uncommon among young men. Here are some potential causes:
1. Urethral Stricture: This is a narrowing of the urethra that can cause a split stream. It can be congenital or due to injury or infection.
2. Foreskin Issues: If you have a tight foreskin (phimosis), it can affect the flow of urine. Your doctor’s advice to retract the foreskin while urinating is a common recommendation, but if you’re still experiencing issues, it might be worth discussing further options with a urologist.
3. Pelvic Floor Muscle Tension: Sometimes, tension in the pelvic floor muscles can affect how urine flows. If these muscles are too tight, they can impede a smooth stream.
4. Positioning: The way you position yourself while urinating can also impact the stream. Standing too far from the toilet or at an awkward angle can lead to a split stream.
Post-Void Leakage
Post-void leakage, or the dripping of urine after you think you’re done, is also a common issue. Here are some factors that might contribute to this:
1. Incomplete Bladder Emptying: If your bladder isn't fully emptying, residual urine can leak out after you’ve finished urinating. This can be related to pelvic floor muscle tension or other anatomical issues.
2. Prostate Health: While prostate issues are more common in older men, younger men can experience conditions like prostatitis, which can lead to urinary symptoms.
3. Nerve Function: The nerves that control bladder function can sometimes be affected by various factors, including stress or anxiety, which can lead to issues like leakage.
Connection to Holding Urine and Masturbation
You mentioned that you sometimes hold your urine and are curious if this could be related to your symptoms. Holding urine for extended periods can lead to bladder overactivity and may contribute to issues like urgency or leakage. It’s generally advisable to urinate when you feel the urge to prevent any potential complications.
As for masturbation, while it’s a normal part of sexual health, it typically does not have a direct impact on urinary function. However, if you notice that your urinary symptoms worsen after masturbation, it might be worth discussing this with a healthcare provider to rule out any underlying issues.
Recommendations
1. Hydration: Ensure you’re drinking enough water throughout the day. Dehydration can concentrate urine and potentially irritate the bladder.
2. Pelvic Floor Exercises: Consider doing pelvic floor exercises (Kegel exercises) to strengthen the muscles involved in urination. This can help with both the split stream and post-void leakage.
3. Follow-Up with a Urologist: Since you’ve already seen a urologist, it might be beneficial to return for a follow-up. Be sure to discuss all your symptoms in detail, including the split stream and leakage. They may recommend further tests, such as a uroflowmetry test, to assess your urinary function more closely.
4. Avoid Holding Urine: Try to urinate when you feel the urge rather than holding it for long periods.
In conclusion, while your symptoms can be concerning, they are often manageable with the right approach. Don't hesitate to seek further medical advice, as a healthcare professional can provide tailored recommendations based on your specific situation. Take care!
