Chronic diarrhea issues?

I have been experiencing intermittent gastrointestinal issues for over a year now! I have had periods of improvement followed by relapses, and recently it has become quite severe.
Sometimes I wake up feeling the urge to have a bowel movement, and I experience cramping abdominal pain.
After using the restroom, I feel somewhat relieved.
However, this ongoing situation has caused me significant psychological stress and burden.
It is particularly distressing to deal with abdominal cramps during important events.
Previous examinations suggested that I might have irritable bowel syndrome (IBS).
Recently, I started taking an over-the-counter intestinal health medication called "Xin Biao Fei Ming," but after more than a week of use, I still experience the urge to have diarrhea.
Therefore, I am unsure how to resolve this situation.
Additionally, is "Xin Biao Fei Ming" a suitable medication? If it is not appropriate, I would appreciate your guidance.
Thank you!

Dr. Chen Shidian reply Gastroenterology and Hepatology

Dear Mr.
A-Qun:
It is recommended that you visit the gastroenterology outpatient clinic for a diagnostic evaluation and medication.
(If you are diagnosed with irritable bowel syndrome, you may request the physician to prescribe long-term medication for occasional use.) Additionally, dietary adjustments and stress management are advised.
Simethicone is effective for bloating, and "charcoal tablets" have a certain efficacy for gas relief.
Changhua Hospital cares about your health.

Sincerely,
Chen Shidian

Managing chronic diarrhea can be a complex challenge, especially when symptoms have persisted for over a year, as you've described. Your experience of alternating between diarrhea and abdominal pain, along with the psychological stress it brings, is not uncommon among individuals with gastrointestinal disorders. Based on your symptoms and previous diagnoses, it seems you may be dealing with a condition known as Irritable Bowel Syndrome (IBS), which is characterized by a combination of abdominal pain and altered bowel habits, including diarrhea and constipation.

Understanding Your Symptoms
1. Chronic Diarrhea: This can be triggered by various factors, including dietary choices, stress, and underlying gastrointestinal conditions. In your case, the fact that you experience relief after bowel movements suggests a functional gastrointestinal disorder, such as IBS, rather than an organic disease.

2. Abdominal Pain: The cramping or "colicky" pain you describe is typical in IBS. This pain often correlates with bowel movements and can be exacerbated by stress or certain foods.

3. Psychological Impact: Chronic gastrointestinal issues can lead to significant anxiety and stress, which can further exacerbate symptoms. It's essential to address both the physical and psychological aspects of your condition.

Treatment Options
1. Dietary Modifications: One of the most effective ways to manage IBS is through dietary changes. Consider keeping a food diary to identify any potential triggers. Common culprits include high-fat foods, dairy, caffeine, and certain carbohydrates known as FODMAPs (fermentable oligosaccharides, disaccharides, monosaccharides, and polyols). A low-FODMAP diet has been shown to help many individuals with IBS.

2. Medications: While you mentioned taking "欣表飛鳴" (which I assume is a probiotic or a gut health supplement), it's essential to consult with a healthcare professional about its efficacy for your specific symptoms. Probiotics can be beneficial for some individuals, but they may not work for everyone. Over-the-counter medications like loperamide (Imodium) can help manage diarrhea but should be used cautiously and not as a long-term solution.

3. Stress Management: Since stress can significantly impact gastrointestinal health, incorporating stress-reduction techniques such as mindfulness, yoga, or cognitive behavioral therapy (CBT) can be beneficial. These approaches can help you manage anxiety related to your symptoms and improve your overall quality of life.

4. Regular Follow-ups: Given the chronic nature of your symptoms, regular follow-ups with a gastroenterologist are crucial. They can provide ongoing support, adjust medications as needed, and recommend further testing if necessary. If your symptoms persist despite treatment, additional evaluations such as stool tests, imaging studies, or even a colonoscopy may be warranted to rule out other conditions.

5. Consider Psychological Support: If you find that your symptoms are significantly impacting your mental health, seeking support from a mental health professional can be beneficial. They can help you develop coping strategies and address any anxiety or depression that may arise from your chronic condition.

Conclusion
In summary, managing chronic diarrhea and associated symptoms requires a multifaceted approach that includes dietary changes, medication management, stress reduction, and regular medical follow-ups. While "欣表飛鳴" may provide some benefits, it's essential to ensure that any treatment plan is tailored to your specific needs and symptoms. Consulting with a healthcare provider who specializes in gastrointestinal disorders can help you navigate this complex condition and improve your quality of life. Remember, you are not alone in this journey, and there are effective strategies to help manage your symptoms and reduce the psychological burden they bring.
