Circumcision
Hello, I went to a hospital to consult about my foreskin and glans.
I wanted to know if my foreskin is too long and if the small red blisters on my glans are a concern.
After the examination, the doctor said that my foreskin is indeed too long and that the condition of my glans is a type of skin disease.
He prescribed me Sinquart cream to apply on my glans.
He also asked me if I wanted to undergo circumcision.
Could you please explain the differences between having a circumcision and not having one, as well as the benefits and drawbacks of circumcision? Thank you very much.
I appreciate it.

Dr. Xu Weikai reply Urology
Jason: Hello, in the past, most doctors advocated for circumcision, citing reasons such as easier hygiene and a reduced risk of cervical cancer for partners.
The drawbacks include decreased protection of the glans, making it more susceptible to injury (e.g., from zippers), and direct exposure of the glans, which can lead to increased sensitivity and a higher likelihood of premature ejaculation immediately after surgery.
The foreskin can also be used for skin grafting; once removed, it is no longer available.
Actually, the previous strong advocacy for circumcision has shifted towards a more conservative approach.
Surgery is now recommended only in the following situations:
1.
Phimosis, where the foreskin cannot be easily retracted to expose the glans.
2.
Frequent inflammation of the foreskin.
3.
Presence of lesions on the foreskin that require excision and biopsy.
4.
Fibrosis of the foreskin (thickening and hardening).
This is for your reference.
Sincerely, Dr.
Hsu Wei-Kai, Director of Urology, Taitung Hospital, Department of Health, Executive Yuan.

More Info
Circumcision is a surgical procedure that involves the removal of the foreskin, which is the fold of skin covering the glans (head) of the penis. This procedure can be performed for various reasons, including medical, religious, or cultural. Understanding the benefits, risks, and alternatives to circumcision is essential for making an informed decision.
Benefits of Circumcision
1. Reduced Risk of Infections: Circumcision has been associated with a lower risk of urinary tract infections (UTIs) in infants and a reduced risk of sexually transmitted infections (STIs) in adults, including HIV. The removal of the foreskin can make it easier to maintain hygiene and reduce the accumulation of bacteria.
2. Prevention of Medical Conditions: Circumcision can prevent certain medical conditions, such as phimosis (a condition where the foreskin cannot be easily retracted) and balanitis (inflammation of the glans). These conditions can cause pain and discomfort, and circumcision can provide a permanent solution.
3. Potential Decreased Risk of Penile Cancer: While penile cancer is rare, studies suggest that circumcision may lower the risk of developing this type of cancer. The exact reasons are not fully understood, but it may be related to improved hygiene and reduced risk of infections.
4. Cultural and Religious Significance: For many, circumcision is an important cultural or religious practice. It can be a rite of passage in certain communities, and for some families, it holds significant spiritual meaning.
Risks of Circumcision
1. Surgical Risks: As with any surgical procedure, circumcision carries risks such as bleeding, infection, and adverse reactions to anesthesia. While these complications are rare, they can occur.
2. Pain and Discomfort: Post-operative pain is common, and while it can be managed with medication, some individuals may experience discomfort during the healing process.
3. Changes in Sensation: Some men report changes in sensitivity after circumcision. While some may find that they experience less sensitivity, others may not notice a significant difference.
4. Psychological Impact: For some individuals, circumcision can have psychological effects, particularly if it is performed later in life. Feelings of loss or trauma may arise, and it is essential to consider the emotional aspects of the decision.
Alternatives to Circumcision
1. Conservative Management: For conditions like phimosis or balanitis, conservative treatments such as topical creams (like the one prescribed to you) or improved hygiene practices can be effective. In many cases, these conditions can resolve without the need for surgery.
2. Regular Monitoring: If there are no significant symptoms or complications, regular monitoring by a healthcare provider may be a suitable alternative. This approach allows for the observation of any changes without immediate intervention.
3. Education and Counseling: Understanding the anatomy, hygiene practices, and potential issues related to the foreskin can help individuals make informed decisions about their health without necessarily resorting to circumcision.
Conclusion
The decision to circumcise should be made after careful consideration of the benefits and risks, as well as personal, cultural, and medical factors. It is essential to have an open discussion with your healthcare provider about your specific situation, including the condition of your foreskin and glans, and to explore all available options. If you have concerns about the prescribed treatment or the need for circumcision, seeking a second opinion from another qualified healthcare professional may also be beneficial. Ultimately, the goal is to ensure your health and well-being while respecting your preferences and values.
