Unexplained redness, swelling, and itching of the feet?

Hello, Dr.
Hong.
For the past few nights, my feet (specifically the front part of all ten toes) have been inexplicably red, swollen, and itchy.
This occurs around the time I go to bed (approximately between 10:30 PM and midnight).
The itching lasts for a while (about 3 to 5 hours) and then subsides, with no itching during the day! When it happens, my toes become completely swollen, and all ten turn red, followed by intense itching.
I can't help but scratch, and I've noticed that the red, swollen areas are painful when pressed, and even the skin around the toenails is tender.
The appearance of my feet looks normal; there are no blisters or other abnormalities—just swelling and redness! I find that applying ice provides some relief from the itching, but I can't keep icing them while trying to sleep.
This happened once last year as well! The episodes are brief and not frequent.
Since that previous experience, I've been very careful about foot hygiene, but I didn't expect this to happen again.
What's even more frustrating is that I don't know the cause, so I can't prevent it! The medication I'm currently using is something I received from a dermatologist after experiencing unexplained itching all over my body in the past, but it doesn't seem to be effective (the previous topical treatments for itching didn't help either).
According to the doctor, there are times when a person's immune system is lower, making them more susceptible to allergies.
The solution was just to wait it out until the itching subsided, but my skin has been in poor condition since then.
Dr.
Hong, please help me! > < May everything go smoothly for you, and may Jesus love you.

Unexplained red, swollen, and itchy toes can be quite distressing, especially when they occur at night and seem to resolve on their own after a few hours. Based on your description, there are several potential causes for your symptoms, and I will outline them along with possible solutions.

Possible Causes
1. Allergic Reaction: One of the most common reasons for sudden redness and swelling is an allergic reaction. This could be due to something you’ve come into contact with, such as a new detergent, soap, or even footwear materials. Allergies can cause localized swelling, redness, and itching.

2. Contact Dermatitis: Similar to an allergic reaction, contact dermatitis occurs when your skin reacts to a substance it has come into contact with. This could be a chemical in a product or even a plant. The symptoms typically include redness, swelling, and itching.

3. Insect Bites: If you have been outdoors, it’s possible that you have been bitten by an insect. Insect bites can cause localized swelling and redness, and they often itch.

4. Fungal Infection: Fungal infections, such as athlete's foot, can cause redness and itching, particularly between the toes. While you mentioned that your toes appear normal otherwise, it's worth considering if there might be a fungal component.

5. Gout: Gout is a type of arthritis that can cause sudden and severe pain, redness, and swelling in the joints, often affecting the big toe. While you mentioned that the pain is not constant, it’s something to consider if you have a history of gout or if you consume foods high in purines.

6. Eczema or Psoriasis: These chronic skin conditions can cause red, itchy patches on the skin, including on the toes. They may flare up due to stress, changes in weather, or irritants.

Solutions
1. Consult a Dermatologist: Since this issue has persisted and recurred, it would be wise to consult a dermatologist. They can provide a thorough examination and may perform patch testing to identify any allergens.

2. Over-the-Counter Treatments: You might consider using over-the-counter antihistamines to help with itching and swelling. Topical corticosteroids can also reduce inflammation and itching. However, be cautious with prolonged use of topical steroids, as they can thin the skin.

3. Cold Compress: As you mentioned that ice packs provide some relief, continue using cold compresses to reduce swelling and soothe itching. Just be sure to wrap ice in a cloth to avoid direct contact with the skin.

4. Maintain Hygiene: Keep your feet clean and dry. If you suspect a fungal infection, antifungal powders or creams might be beneficial. Ensure that your shoes are breathable to prevent moisture buildup.

5. Avoid Known Irritants: If you suspect that a product or material may be causing your symptoms, try to eliminate it from your routine. This includes soaps, lotions, and even certain fabrics.

6. Monitor Your Diet: If gout is a concern, monitor your diet for high-purine foods such as red meat, shellfish, and alcohol. Staying hydrated can also help reduce the risk of gout attacks.

7. Stress Management: Since stress can exacerbate skin conditions, consider incorporating stress-reducing activities into your routine, such as yoga, meditation, or regular exercise.

Conclusion
While the symptoms you are experiencing can be uncomfortable and concerning, there are various potential causes and solutions to explore. It is essential to seek professional medical advice for a definitive diagnosis and tailored treatment plan. In the meantime, maintaining good foot hygiene, using cold compresses, and avoiding potential irritants may help alleviate your symptoms. Remember, you are not alone in this, and with the right approach, you can find relief.
