Foreskin issues

Hello doctor, I have developed many small white spots on my foreskin, probably several hundred.
I’m not sure what they are; they don't hurt or itch, and they have been there for a long time, about 2 to 3 years.
I also have no sexual experience.
Could you tell me what this might be? I looked online and wondered if it could be "Lichen Sclerosus." How should it be treated? If it’s not that, what condition do I have? Which specialist should I see? Thank you.

A-Qing, 10~19 year old female. Ask Date: 2007/02/21

Dr. Xu Weikai reply Urology

A-Qing: Hello, Happy New Year! I'm not sure what that is, but it could be a sebaceous cyst, which occurs when a sebaceous gland becomes blocked, resulting in something similar to acne.
When squeezed hard, it may sometimes release a white liquid or powdery substance.
Improving local hygiene should be effective.
If you're still concerned, you can take a picture and bring it to the clinic for the doctor to review.
I apologize for the delayed response due to the New Year holiday.
Sincerely, Dr.
Hsu Wei-Kai, Director of Urology, Taitung Hospital, Department of Health, Executive Yuan.

The presence of white bumps on the foreskin can be concerning, especially when they persist for an extended period, as in your case of 2-3 years. Based on your description, these bumps are likely to be pearly penile papules or Fordyce spots, which are common and benign conditions.
Pearly Penile Papules are small, dome-shaped bumps that typically appear around the corona of the glans (the head of the penis). They are usually white or skin-colored and are not associated with any pain, itching, or other symptoms. These papules are a normal anatomical variation and are not a sign of any disease or infection. They are harmless and do not require treatment unless they cause psychological distress or concern.

Fordyce Spots, on the other hand, are small, painless, pale bumps that can appear on the shaft of the penis or the scrotum. They are sebaceous glands that are visible due to their size and are also completely benign. Like pearly penile papules, they do not require treatment.

Given that you have mentioned that these bumps do not cause any pain or itching and have been present for a long time, it is unlikely that they are a sign of a sexually transmitted infection (STI) or any other serious condition. Conditions like genital warts, which are caused by the human papillomavirus (HPV), typically present differently, often with additional symptoms such as itching or discomfort.

However, if you are concerned about the appearance of these bumps or if there are any changes in their characteristics (such as color, size, or if they start to itch or become painful), it would be wise to consult a healthcare professional.
In terms of which specialist to see, a dermatologist would be the most appropriate choice, as they specialize in skin conditions and can provide a thorough examination and diagnosis. If you prefer, a urologist is also qualified to address concerns related to the male reproductive system and can help rule out any other potential issues.

In summary, while the white bumps you are experiencing are likely benign and common, it is always best to consult a healthcare professional for an accurate diagnosis and peace of mind. They can provide you with the necessary information and, if needed, discuss treatment options, although in most cases, no treatment is required for these conditions.
