Can I Get Pregnant If I Missed a Birth Control Pill? - Obstetrics and Gynecology

Could this lead to pregnancy?

Doctor, I have been taking a 21-day birth control pill, usually around 11 or 12 PM.
My last menstrual period was from February 21 to February 27.
On February 28, I had intercourse with my boyfriend, and he ejaculated inside me.
I started the second cycle of birth control pills on March 1 (the break period for the first cycle was from February 22 to February 28).
However, I forgot to take my pill on March 3, and when I realized it on March 4, it was already 9:30 PM, so I took two pills at once (including the one I missed).
I would like to know if this increases my risk of pregnancy.
Additionally, can I continue taking my pills at the usual time of 11 or 12 PM after March 5? Thank you for your help.

Hello: No contraceptive method is 100% effective, especially since you did not start taking the birth control pills from the third day of your menstrual cycle (February 23) and missed a dose on March 3.
This will certainly reduce the effectiveness of contraception.

Missing a birth control pill can indeed raise concerns about the possibility of becoming pregnant, especially if you have had unprotected intercourse around the time of the missed pill. In your case, you were taking a 21-day birth control pill regimen and missed a pill on March 3rd, taking two pills the following day to catch up. Here’s a breakdown of the situation and some important considerations regarding your question.

Understanding Birth Control Pills
Birth control pills work primarily by preventing ovulation. They contain hormones that regulate your menstrual cycle and inhibit the release of eggs from the ovaries. When taken consistently and correctly, they are highly effective at preventing pregnancy—over 99% effective. However, missing a pill can compromise this effectiveness.

What Happens When You Miss a Pill?
1. Timing of the Missed Pill: You missed a pill during the second week of your cycle. According to most guidelines, if you miss a pill during the second week and take it as soon as you remember (which you did by taking two pills the next day), your protection against pregnancy is generally maintained.
2. Unprotected Intercourse: You mentioned having unprotected intercourse on February 28th. Since you were still in the first week of your new pill cycle, you were likely still protected from pregnancy, provided you had taken your pills correctly during the first week. However, since you missed a pill in the second week, there is a slight risk of ovulation occurring, which could lead to pregnancy.

3. Emergency Contraception: If you are concerned about the risk of pregnancy due to the missed pill and the timing of your intercourse, you might consider using emergency contraception (EC) if it’s within the appropriate time frame (typically within 72 to 120 hours after unprotected intercourse). EC is more effective the sooner it is taken.

Continuing Your Birth Control Regimen
You asked whether you can continue taking your pills at the same time (around 11 or 12 PM) after the missed pill incident. Yes, you can continue taking your pills at your usual time. Consistency is key with birth control pills, so try to take them at the same time every day to maintain their effectiveness. If you find that you often forget to take your pill, consider setting an alarm or using a pill organizer to help you remember.

Conclusion
In summary, while missing a birth control pill does increase the risk of pregnancy, your actions to take the missed pill as soon as you remembered help mitigate that risk. Given that you had unprotected intercourse before the missed pill but were still in the first week of your new cycle, you may still have been protected. However, if you have any concerns or experience any unusual symptoms, it’s always best to consult with your healthcare provider for personalized advice and to discuss the option of emergency contraception if you feel it’s necessary.

Remember, effective communication with your healthcare provider about your birth control method and any concerns you have is crucial for your reproductive health.
