My boyfriend?

Doctor, my boyfriend has been experiencing intense itching in his genital area.
He mentioned that there is a horizontal crack beneath his glans.
After having intercourse once, he noticed bleeding from the crack, which is how he discovered it.
Although it is no longer bleeding, he still feels itchy, and the crack is still present.
What could be the cause of this? Thank you.

DORIS, 20~29 year old female. Ask Date: 2007/03/22

Dr. Lai Ronghao reply Urology

Hello: Based on what you described, it could possibly be pruritus, eczema, or chronic dermatitis.
The cracks may be scar tissue resulting from an injury.
If you continue to feel discomfort or have any issues, please visit a hospital for evaluation.
Thank you!

Itchy symptoms and cracks in the male genitalia can be concerning and may indicate a variety of underlying conditions. Based on your description, it sounds like your boyfriend is experiencing a combination of symptoms that could be attributed to several potential causes.

One common cause of itching and cracking in the genital area is a condition known as balanitis, which is inflammation of the glans (the head of the penis). Balanitis can be caused by a variety of factors, including poor hygiene, infections (such as yeast infections or sexually transmitted infections), or skin conditions like eczema or psoriasis. The presence of a horizontal crack, especially if it has bled, could suggest that the skin has become irritated or damaged, possibly due to friction during sexual activity or an underlying infection.

Another possibility is a condition called lichen sclerosus, which is a chronic inflammatory skin condition that can affect the genital area. It often presents as white patches of skin, itching, and can lead to scarring and fissures. This condition is more common in uncircumcised males and can cause significant discomfort.

Sexually transmitted infections (STIs) such as herpes or syphilis can also present with symptoms of itching and lesions. If your boyfriend has had unprotected sexual contact, it would be prudent to consider this possibility and seek testing.

Additionally, contact dermatitis, which is an allergic reaction to soaps, lotions, or other products that come into contact with the genital area, could also be a contributing factor. This condition can lead to itching, redness, and sometimes cracking of the skin.

Given that the crack has bled and is still present, it is essential for your boyfriend to seek medical attention. A healthcare provider can perform a physical examination, take a detailed history, and possibly conduct tests to determine the exact cause of the symptoms. Treatment will depend on the underlying cause but may include topical corticosteroids for inflammation, antifungal creams for yeast infections, or other specific treatments based on the diagnosis.

In the meantime, it may help for your boyfriend to keep the area clean and dry, avoid irritants such as scented soaps or lotions, and refrain from sexual activity until he has been evaluated by a healthcare professional. This will help prevent further irritation and allow any potential healing to occur.

In summary, while there are several potential causes for the symptoms your boyfriend is experiencing, it is crucial to consult a healthcare provider for an accurate diagnosis and appropriate treatment. Early intervention can help alleviate discomfort and prevent complications.
