There is a wheezing sound when breathing during sleep?

Hello, doctor.
Recently, I've noticed that when I lie down to sleep, I sometimes hear a wheezing sound while breathing, which is accompanied by coughing.
At times, I feel like I have difficulty breathing.
I have visited three family medicine clinics, and while the medications provided some relief, the symptoms returned shortly after.
I still experience coughing and wheezing, but these symptoms are particularly pronounced before sleeping and upon waking up.
Could you please tell me what might be causing these symptoms? Is it asthma? I've had these symptoms for about two months without improvement, and there is no family history of asthma or similar conditions.
Which specialist should I consult? Thank you for your help!

Mr. Hong, 20~29 year old female. Ask Date: 2007/04/08

Hello! It sounds like you are experiencing some troubling symptoms related to wheezing and coughing, particularly at night. These symptoms can be quite distressing and may indicate an underlying respiratory issue. Let's break down the possible causes and solutions for your situation.

Wheezing and coughing, especially when they occur at night or upon waking, can be indicative of several conditions. One of the most common causes is asthma, a chronic inflammatory disease of the airways that can lead to wheezing, coughing, shortness of breath, and chest tightness. Although you mentioned that there is no family history of asthma, it is still possible for individuals to develop asthma without a familial predisposition. Asthma symptoms can be triggered by various factors, including allergens (like dust mites, pet dander, or pollen), irritants (such as smoke or strong odors), respiratory infections, or even exercise.

Another potential cause of your symptoms could be gastroesophageal reflux disease (GERD). When lying down, stomach acid can flow back into the esophagus, leading to irritation and triggering a cough reflex. This can also cause wheezing if the acid irritates the airways. If you notice that your symptoms worsen after eating or when lying down, GERD might be a contributing factor.

Allergic reactions can also lead to nighttime coughing and wheezing. If you have allergies, exposure to allergens in your bedroom, such as dust mites or mold, could exacerbate your symptoms at night. Consider evaluating your sleeping environment for potential allergens and taking steps to minimize exposure.

Given that you have seen multiple family medicine doctors and experienced temporary relief with medication, it may be beneficial to consult with a specialist. An allergist or a pulmonologist would be appropriate choices. An allergist can conduct tests to determine if allergies are contributing to your symptoms, while a pulmonologist specializes in respiratory conditions and can perform tests such as spirometry to assess lung function and diagnose asthma or other pulmonary issues.

In the meantime, here are some strategies you can implement to help alleviate your symptoms:
1. Environmental Control: Ensure your sleeping area is free from allergens. Use hypoallergenic bedding, wash your sheets regularly in hot water, and consider using an air purifier.

2. Sleep Position: If you suspect GERD, try sleeping with your head elevated to prevent acid reflux.
3. Avoid Triggers: Identify and avoid any known triggers for your symptoms, whether they are environmental, dietary, or related to stress.

4. Medication: If you have been prescribed medication, ensure you are taking it as directed. If your symptoms persist, follow up with your doctor to discuss alternative treatments or adjustments to your current regimen.

5. Monitor Symptoms: Keep a diary of your symptoms, noting when they occur, their severity, and any potential triggers. This information can be valuable for your healthcare provider in diagnosing and managing your condition.

In conclusion, while your symptoms may be indicative of asthma or another respiratory condition, a thorough evaluation by a specialist is essential for an accurate diagnosis and effective treatment plan. Don't hesitate to seek further medical advice, as managing respiratory symptoms early can significantly improve your quality of life. Take care, and I hope you find relief soon!
