Sexual function

I have always had this issue that I don't understand.
Is there really a clinical case of premature ejaculation? Specifically, ejaculation occurs after only a brief stimulation of the penis, within 10 to 20 seconds.
I hope the doctor can provide some answers.
If I have such symptoms, do I need medication, or I have seen online about something called retrograde nerve therapy? Thank you.

DD, 10~19 year old female. Ask Date: 2007/05/06

Dr. Xu Weikai reply Urology

DD: Hello, premature ejaculation, from an evolutionary biology perspective, is a normal phenomenon.
All animals are most vulnerable during two times: 1.
sleeping; and 2.
mating.
During these periods, they are most susceptible to attacks.
Therefore, from an evolutionary standpoint, premature ejaculation may actually be a trait that enhances survival.
Later on, as humans developed shelters for protection, these two issues became less of a concern, leading to longer sexual intercourse.
At your age, premature ejaculation is considered normal.
Over the years, it will gradually improve.
Additionally, using condoms throughout intercourse can help alleviate most cases of premature ejaculation.
Furthermore, the duration of masturbation is generally shorter.
This is a response from Dr.
Hsu Wei-kai, Director of Urology at Taitung Hospital, Department of Health, Executive Yuan.

Premature ejaculation (PE) is a common sexual dysfunction that affects many men at some point in their lives. It is characterized by the inability to control ejaculation, leading to ejaculation occurring sooner than desired, often within a minute of penetration or even before it. This condition can cause significant distress and impact relationships, leading many to seek treatment options.

Understanding the Causes of Premature Ejaculation
The causes of PE can be broadly categorized into psychological and biological factors. Psychological factors may include anxiety, stress, depression, or relationship issues. Many men may experience performance anxiety, which can exacerbate the problem. On the biological side, factors such as hormonal imbalances, neurotransmitter levels, and even genetic predispositions can play a role. Conditions like prostatitis or hyperthyroidism may also contribute to PE.

Diagnosis of Premature Ejaculation
To diagnose PE, healthcare providers typically conduct a thorough medical history and physical examination. They may ask about the duration of the problem, the frequency of episodes, and any associated symptoms. It is essential to differentiate between lifelong PE (present since the first sexual experiences) and acquired PE (developing later in life), as treatment approaches may vary.

Treatment Options for Premature Ejaculation
1. Behavioral Techniques: Many men find success with behavioral techniques such as the "start-stop" method or the "squeeze" technique. These methods involve training the body to delay ejaculation through controlled stimulation.

2. Topical Anesthetics: Over-the-counter creams or sprays containing lidocaine or benzocaine can be applied to the penis to reduce sensitivity and help delay ejaculation. These products should be used cautiously to avoid transferring the anesthetic to a partner.

3. Oral Medications: Selective serotonin reuptake inhibitors (SSRIs), commonly used as antidepressants, have been found effective in delaying ejaculation. Medications such as paroxetine, sertraline, and fluoxetine may be prescribed off-label for this purpose. Another option is the use of tramadol, a pain medication that has also shown efficacy in delaying ejaculation.

4. Counseling and Therapy: If psychological factors are contributing to PE, counseling or sex therapy may be beneficial. A therapist can help address anxiety, relationship issues, and other underlying psychological concerns.

5. Advanced Treatments: In some cases, men may consider more advanced treatments such as nerve blocks or even surgical options. However, these are typically reserved for severe cases that do not respond to other treatments.

6. Reverse Nerve Therapy: This is a less common approach that involves techniques aimed at altering nerve pathways to improve control over ejaculation. However, the efficacy and safety of such treatments are still under investigation, and they may not be widely available.

Risks and Considerations
While many treatments for PE are effective, they may come with potential side effects. For instance, SSRIs can lead to decreased libido, fatigue, or gastrointestinal issues. Topical anesthetics may cause skin irritation or numbness. It is crucial to discuss these risks with a healthcare provider to determine the best treatment plan tailored to individual needs.

Conclusion
If you are experiencing symptoms of premature ejaculation, it is essential to consult a healthcare professional for a proper evaluation and treatment plan. PE is a common issue, and various effective treatment options are available. Addressing the condition can significantly improve sexual satisfaction and overall quality of life. Remember, open communication with your partner and healthcare provider is key to managing this condition effectively.
