Pressing on the upper left abdomen causes pain?
Dear Dr.
Lin Ming-Hui,
I would like to ask you about some symptoms I have been experiencing recently.
Since March and April of last year, I have noticed alternating bowel movements, sometimes having a solid stool and other times experiencing diarrhea.
Occasionally, there is mucus present, and I have been going to the bathroom 2 to 3 times a day, or sometimes just once.
In the last few months, I have observed small black particles in my stool, which are not as small as black specks, even though I haven't consumed any black foods in the days leading up to this.
However, there is no fresh blood in my stool.
Additionally, I often feel discomfort in my upper left abdomen upon waking, but it improves significantly after a bowel movement.
On days when I do not have a bowel movement, I experience some abdominal bloating.
Recently, I have not been able to have regular bowel movements, and my abdomen feels bloated.
Pressing on my upper left abdomen also causes some pain.
I would like to know what might be happening to me.
Thank you for your response.
I appreciate it greatly.

Dr. Lin Minghui reply Gastroenterology and Hepatology
You are experiencing symptoms of abnormal bowel movements, bloating, and abdominal pain, which improve after defecation.
If you do not have a proper bowel movement, your abdomen feels bloated.
These symptoms are consistent with a diagnosis of irritable bowel syndrome (IBS).
Stress and diet can influence IBS.
Non-pharmacological treatments include relaxation techniques and dietary modifications, such as reducing the intake of spicy and irritating foods, including those containing caffeine.
If you frequently experience bloating, it is advisable to limit foods that can cause gas, such as milk, beans, and dairy products.

More Info
Dear patient,
Thank you for reaching out with your concerns regarding your left upper abdominal pain and digestive issues. Based on the symptoms you've described, it seems that you are experiencing a combination of gastrointestinal disturbances that could be related to several underlying conditions. Let's break down your symptoms and explore potential causes and recommendations.
Symptoms Overview
1. Bowel Movements: You mentioned alternating between normal and loose stools, sometimes accompanied by mucus, and experiencing black specks in your stool. The presence of black material in your stool can be indicative of gastrointestinal bleeding, particularly from the upper gastrointestinal tract, which may result from conditions such as peptic ulcers or gastritis. However, it’s crucial to note that certain foods, medications (like iron supplements), or even supplements can also cause dark stools.
2. Abdominal Discomfort: The discomfort in your left upper abdomen, particularly after waking up and the relief you feel after bowel movements, suggests a possible connection between your digestive health and the pain you are experiencing. The fact that your pain improves after defecation may indicate that your symptoms are related to bowel function, possibly involving the colon or the intestines.
3. Bloating and Pain: The sensation of bloating and pain in the left upper abdomen, especially when pressing on the area, could be due to several factors, including gas buildup, constipation, or even issues related to the spleen or stomach.
Potential Causes
Given your symptoms, several conditions could be contributing to your discomfort:
- Irritable Bowel Syndrome (IBS): This functional gastrointestinal disorder can cause alternating bowel habits, bloating, and abdominal pain. Stress and dietary factors often exacerbate symptoms.
- Gastrointestinal Bleeding: The presence of black specks in your stool warrants further investigation to rule out bleeding from the upper GI tract.
- Peptic Ulcers or Gastritis: These conditions can cause abdominal pain and discomfort, especially after meals, and may lead to changes in bowel habits.
- Diverticulitis: Inflammation of diverticula in the colon can cause localized pain and changes in bowel habits.
Recommendations
1. Seek Medical Attention: It is essential to consult a healthcare provider for a thorough evaluation. Given the presence of black material in your stool, a gastroenterologist may recommend diagnostic tests such as an upper endoscopy or colonoscopy to assess for ulcers, gastritis, or other abnormalities.
2. Dietary Modifications: Keeping a food diary can help identify any potential triggers for your symptoms. Avoiding high-fat, spicy, or irritating foods may alleviate some discomfort. Increasing fiber intake gradually can help regulate bowel movements.
3. Hydration: Ensure you are drinking plenty of fluids, as dehydration can worsen constipation and abdominal discomfort.
4. Stress Management: Since stress can exacerbate gastrointestinal symptoms, consider incorporating stress-reduction techniques such as mindfulness, yoga, or regular exercise.
5. Medications: If you have been prescribed medications for digestive issues, ensure you are taking them as directed. Over-the-counter options like antacids or probiotics may also provide relief, but consult your doctor before starting any new medication.
Conclusion
Your symptoms warrant a comprehensive evaluation to determine the underlying cause and appropriate treatment. It is crucial not to ignore the signs your body is presenting, especially concerning changes in bowel habits and abdominal pain. Please make an appointment with your healthcare provider as soon as possible to discuss your symptoms in detail and undergo any necessary testing.
Thank you for your inquiry, and I wish you the best in your journey toward better health.
