Help! During my workout, I suddenly experienced an ejaculation?
Hello, doctor.
Yesterday, I worked out at the gym and did a pull-down exercise on a machine where you pull down from above to the neck area.
The weight I was using wasn't very heavy, about 5 plates.
I started with 20 repetitions, then did sets of 10 with a 30-second to one-minute rest in between.
By the time I reached the fourth set of 10 repetitions, I started to feel something unusual in my penis.
It wasn't an erection, but more like a twitching sensation.
When I sat up straight and began the fifth set of 10 repetitions, I unexpectedly ejaculated.
Is this a common occurrence? In middle school, when we ran on the field for a long time, I also experienced similar sensations.
It would happen when I ran very fast.
However, recently, when I run on the treadmill for 15-20 minutes, I don't feel this way at all.
I really want to know why this sudden ejaculation happened.
I held on until the last set and then ejaculated.
Is this a medical issue? I'm quite worried.

Dr. Xu Weikai reply Urology
Jason: Hello, erection and ejaculation are related to the sympathetic and parasympathetic nervous systems.
Some individuals may experience ejaculation during intense physical activity, although this occurrence is quite rare.
Additionally, sometimes spinal compression can lead to nerve excitation during exercise, which may also result in ejaculation.
In such cases, it is advisable to seek medical attention.
A check-up to see if there is any nerve compression (such as from a tumor) would provide more peace of mind.

More Info
Experiencing unexpected ejaculation during exercise can be both surprising and concerning. This phenomenon, while not commonly discussed, can occur due to several physiological and psychological factors. Let's delve into the potential causes and considerations surrounding this issue.
Possible Causes
1. Physical Stimulation: Certain exercises, especially those that engage the core and pelvic muscles, can inadvertently stimulate the genital area. For instance, exercises that involve pulling motions or those that require significant core engagement may lead to sensations that can trigger ejaculation. The pelvic floor muscles are closely linked to sexual function, and their contraction during exercise can sometimes lead to unexpected ejaculation.
2. Nervous System Response: The body’s autonomic nervous system, which controls involuntary bodily functions, plays a role in sexual arousal and ejaculation. During intense physical activity, the sympathetic nervous system is activated, which can lead to heightened sensitivity and involuntary responses, including ejaculation.
3. Hormonal Fluctuations: Exercise can influence hormone levels in the body, including testosterone, which is linked to sexual arousal. A sudden increase in testosterone during or after exercise might contribute to unexpected ejaculation.
4. Psychological Factors: Anxiety, excitement, or even the physical exertion itself can create a heightened state of arousal. If you are particularly focused on your performance or feeling anxious about the workout, this could lead to unexpected physiological responses.
5. Previous Experiences: As you mentioned, similar experiences during physical activities in the past, such as running, suggest that your body may have a predisposition to respond in this way under certain conditions. This could be a learned response where the body associates physical exertion with sexual arousal.
Concerns and Considerations
While occasional unexpected ejaculation during exercise is generally not a cause for alarm, it can be concerning if it happens frequently or if it interferes with your ability to work out comfortably. Here are some considerations:
- Frequency: If this is a one-time occurrence, it may not warrant concern. However, if it happens regularly, it might be worth discussing with a healthcare professional to rule out any underlying issues.
- Impact on Exercise: If this response is affecting your ability to engage in physical activity or causing embarrassment, it may be beneficial to modify your workout routine. Consider exercises that do not involve significant pelvic engagement or that allow for more control over your body’s responses.
- Consultation with a Specialist: If you find this issue distressing or if it continues to occur, consulting a urologist or a healthcare provider specializing in sexual health may provide you with more personalized insights and recommendations.
Conclusion
Unexpected ejaculation during exercise can stem from a variety of physical and psychological factors. While it is often not a serious issue, understanding the underlying causes can help alleviate concerns. If this experience persists or causes significant distress, seeking professional advice is a prudent step. Remember, open communication with healthcare providers can lead to better understanding and management of such experiences.
