Myocardial infarction
Hello Dr.
Huang, my father underwent a coronary angioplasty with stent placement in mid-March due to a myocardial infarction.
During the postoperative period, his chest pain symptoms significantly improved.
However, recently he has been experiencing shortness of breath.
Initially, he would only feel breathless when going up and down stairs, but now he occasionally experiences the same symptoms even while sitting in a chair.
Is this a normal phenomenon following the procedure, or could there be another underlying cause? I would greatly appreciate your guidance, thank you.

Dr. Huang Guosheng reply Cardiology
Hello Tony, based on your father's symptoms, he is likely experiencing heart failure.
The factors that can exacerbate heart failure include another myocardial infarction, anemia, tachycardia or bradycardia, infection, or worsening renal function.
It is advisable to go to the hospital for further examination and management.

Post-heart attack recovery can be a complex process, and experiencing shortness of breath after stent placement is not uncommon. However, it is essential to understand the underlying causes and whether this symptom is a normal part of recovery or indicative of a more serious issue.
After a heart attack, the heart muscle may be weakened, and the body may take time to adjust to the changes in blood flow and heart function. Stent placement is intended to restore blood flow through the coronary arteries, which can alleviate symptoms like chest pain. However, the recovery process can vary significantly among individuals, and some may experience new or persistent symptoms, including shortness of breath.
In your father's case, the fact that he initially experienced improvement in chest pain but is now facing shortness of breath, even at rest, warrants further investigation. Here are several potential reasons for this symptom:
1. Heart Function: After a heart attack, the heart's ability to pump blood effectively may be compromised. This can lead to heart failure symptoms, including shortness of breath, especially during exertion or even at rest in more severe cases. An echocardiogram can help assess the heart's pumping ability (ejection fraction) and identify any regional wall motion abnormalities.
2. Fluid Retention: If the heart is not pumping efficiently, fluid can accumulate in the lungs, leading to pulmonary congestion. This condition can cause shortness of breath, particularly when lying down or during physical activity. Monitoring for signs of fluid retention, such as swelling in the legs or sudden weight gain, is crucial.
3. Respiratory Issues: Conditions such as pneumonia, pleuritis, or chronic obstructive pulmonary disease (COPD) can also cause shortness of breath. Given that your father has experienced recent respiratory symptoms, a thorough evaluation of lung function may be necessary.
4. Anxiety and Stress: Emotional factors can also contribute to feelings of breathlessness. Anxiety, especially after a significant health event like a heart attack, can manifest as physical symptoms, including shortness of breath.
5. Medication Side Effects: If your father has been prescribed medications post-stent placement, some may have side effects that impact breathing. It’s essential to review his medications with his healthcare provider to ensure they are not contributing to his symptoms.
Given these possibilities, it is crucial for your father to follow up with his healthcare provider. A comprehensive evaluation, including a physical examination, echocardiogram, and possibly pulmonary function tests, can help determine the cause of his shortness of breath.
In the meantime, encourage your father to monitor his symptoms closely. If he experiences worsening shortness of breath, chest pain, or any other concerning symptoms, he should seek immediate medical attention.
In summary, while some degree of shortness of breath can be expected after a heart attack and stent placement, persistent or worsening symptoms should not be ignored. A thorough evaluation is necessary to rule out any complications and ensure appropriate management.
