Leg length discrepancy
My son is 5 years old and has recently been complaining of leg pain, primarily in the lower limbs.
Initially, I thought he was just making it up, but I've noticed that he seems to walk with a limp, and his right leg appears to be shorter.
When I had him stand straight for an examination, it seems like there is indeed a pelvic tilt! I'm very worried and don't know what to do.
Could you please advise me on which medical specialty to consult if I take him to the hospital? Thank you.

Dr. Wang Jiayi reply Pediatrics
Dear concerned mother: May I ask if the painful area on the foot is red, swollen, or warm? Is the pain persistent or does it occur under certain circumstances? I recommend that you take him to a general pediatrics or pediatric orthopedics department at a medical center as soon as possible! Hualien Hospital Pediatrics cares about you! Wishing you a happy Dragon Boat Festival!

When it comes to limb length discrepancy in children, it is understandable for parents to feel concerned, especially when they observe signs such as limping or complaints of pain. Your son's situation, where he frequently complains of leg pain and appears to have a noticeable difference in leg length, warrants a thorough evaluation.
Limb length discrepancy can arise from various causes, including congenital conditions, growth plate issues, or even postural problems. In your case, the observation of pelvic tilt suggests that there may be an underlying structural issue that needs to be addressed. It's important to note that while some degree of limb length discrepancy is common in children, significant differences can lead to functional problems, such as gait abnormalities and discomfort.
Given your son's symptoms, the first step is to consult a pediatric orthopedic specialist. This specialist is trained to assess musculoskeletal issues in children and can provide a comprehensive evaluation. During the visit, the doctor will likely perform a physical examination, which may include measuring the lengths of both legs and assessing your son's gait. They may also check for any signs of scoliosis or other postural abnormalities that could be contributing to the issue.
In some cases, imaging studies such as X-rays may be necessary to evaluate the growth plates and the overall structure of the bones. This can help determine if there are any underlying conditions affecting bone growth. If the discrepancy is confirmed, the orthopedic specialist will discuss potential treatment options based on the severity of the discrepancy and your son's age.
Treatment options can vary widely. In mild cases, observation and regular follow-ups may be sufficient, especially if your son is still growing. In more significant cases, interventions may include physical therapy to strengthen the muscles around the affected limb, orthotic devices to help with alignment, or in some cases, surgical options to correct the discrepancy.
It is also essential to monitor your son's pain levels and any changes in his ability to walk or engage in physical activities. Keeping a record of when the pain occurs, its intensity, and any activities that seem to exacerbate it can be helpful for the doctor during the evaluation.
In summary, your son's symptoms of leg pain and the observation of a potential limb length discrepancy should be taken seriously. A visit to a pediatric orthopedic specialist is the best course of action to ensure a proper diagnosis and to discuss appropriate treatment options. Early intervention can help prevent further complications and support your son's overall development and well-being. Remember, as a parent, your observations and concerns are vital in guiding your child's healthcare journey.
