Postpartum issues
Hello, doctor.
It has been over two months since I gave birth (on April 30).
The lochia cleared up around 42 days postpartum, but then (about 60 days postpartum) I experienced a small amount of red and brown discharge, approximately 5cc per day, which lasted for 7 to 8 days.
Today, on July 10, I have again noticed a small amount of red discharge.
Is this still lochia, or could it be my menstrual period? This abnormal phenomenon is causing me a lot of concern.
Please help clarify this for me.
Thank you.

Hello, thank you for your inquiry.
I would like to address your question and provide the following advice: Generally, postpartum hemorrhage occurs earlier and is usually significant in volume.
As you mentioned, it may be due to poor uterine contractions or hormonal imbalance.
I recommend that you visit an obstetrician-gynecologist for an examination.

Postpartum concerns, particularly regarding abnormal discharge after delivery, can be a source of anxiety for many new mothers. In your case, you mentioned that you are over two months postpartum and experienced a return of red and brown discharge after your lochia (the vaginal discharge after childbirth) had cleared around 42 days. This situation raises several important points to consider.
Firstly, it is essential to differentiate between lochia and the onset of menstruation. Lochia typically consists of blood, mucus, and uterine tissue, and it can last for several weeks postpartum, gradually changing in color and consistency. Initially, lochia is bright red (lochia rubra), transitioning to pink or brown (lochia serosa), and finally to a yellowish-white (lochia alba). The discharge you experienced around 60 days postpartum, which included red and brown secretions, could potentially be a continuation of lochia, especially if it is light and not accompanied by other concerning symptoms.
However, the return of bright red blood after a period of absence raises the possibility of your menstrual cycle returning. It is not uncommon for women who are breastfeeding to experience delayed menstruation, but once breastfeeding frequency decreases or if you are supplementing with formula, your menstrual cycle may resume. The fact that you described the discharge as being minimal (about 5cc per day) could suggest that it is not a typical menstrual flow, which usually presents with heavier bleeding.
There are several factors that could contribute to this abnormal discharge:
1. Hormonal Changes: After childbirth, your body undergoes significant hormonal shifts as it adjusts to non-pregnant status. These hormonal changes can lead to irregular bleeding or spotting.
2. Uterine Healing: If your uterus is still in the process of healing, it might release some blood or tissue, which could be mistaken for a menstrual period.
3. Infection: If the discharge has a foul odor, is accompanied by fever, or if you experience increased pain, it could indicate an infection, such as endometritis, which requires medical attention.
4. Retained Products of Conception: In some cases, remnants of the placenta or other tissues may remain in the uterus, leading to irregular bleeding.
Given your concerns, it is advisable to consult with your healthcare provider. They can perform a thorough examination, possibly including an ultrasound, to rule out any complications such as retained products or infection. Additionally, they can provide guidance on what to expect regarding your menstrual cycle as your body continues to recover from childbirth.
In summary, while some irregular discharge can be normal postpartum, particularly as your body adjusts, it is crucial to monitor the characteristics of the discharge and any accompanying symptoms. If you have any doubts or if the discharge changes in volume, color, or odor, seeking medical advice is the best course of action to ensure your health and peace of mind.
