Insufficient Congestion and Hormone Replacement Issues
Dear Dr.
Hsu,
I have long admired your enthusiastic and detailed responses to patients' various concerns on the online hospital platform.
I would like to seek your guidance and hope you can quickly help me resolve my confusion.
I am generally healthy, do not smoke or drink, enjoy exercising, and have a fulfilling marital life.
However, over the past 1 to 2 years, I have been experiencing significant issues with my sexual function.
Specifically, I have been facing persistent erectile difficulties, with insufficient blood flow leading to a lack of firmness, which is quite discouraging.
I have visited urology and sexual dysfunction clinics multiple times, but the process and results have been disappointing, yielding no effective treatment.
It seems that the doctors have not addressed my specific issues with targeted prescriptions; they only prescribed hormone treatments that last only a few days and provide no real sensation.
This situation has persisted for several years without improvement, leaving me extremely confused.
Can this condition be cured? How can I supplement my hormone levels to return to normal? Is it possible to purchase these supplements over the counter, or are there benefits covered by health insurance? I sincerely hope you can clarify these issues for me and provide guidance.
If you could recommend a good physician, I would be immensely grateful.
Best wishes for your continued success.
Sincerely,

M4: Hello, you are too kind.
I am merely sharing my personal medical experience for patients' reference; it's all about exchanging experiences and growing together.
Generally, most male hormone preparations, aside from a few that have received FDA approval, carry a risk of carcinogenicity.
Therefore, I usually recommend these types of medications only for patients with genuine hormonal deficiencies during puberty that affect their development.
For the average patient, I personally do not recommend their use.
Moreover, most health insurance plans do not cover these medications.
In older individuals, erectile dysfunction is often related to vascular issues, particularly venous leakage.
If treatment is necessary, one can visit a larger hospital for a penile cavernosography to check for vascular leakage.
In severe cases, a minor surgical procedure can be performed to ligate the excess refluxing veins, which tends to yield good results.
Additionally, if the condition is not severe, medications like Viagra can be used to increase arterial blood flow, and they have shown good clinical efficacy.
However, one must be cautious of the side effects of Viagra, especially with first-time use.
These are my suggestions, and I hope they are helpful to you.
Sincerely,
Dr.

Erectile dysfunction (ED) can be a complex issue, often linked to hormonal imbalances, psychological factors, and physical health conditions. Understanding the underlying causes of your symptoms is crucial for effective treatment.
Hormonal Imbalance and Erectile Dysfunction
Hormones play a significant role in sexual function. Testosterone, the primary male sex hormone, is essential for libido and erectile function. Low testosterone levels can lead to reduced sexual desire, erectile difficulties, and other health issues. Other hormones, such as thyroid hormones and prolactin, can also impact sexual health.
In your case, it seems that you have experienced a gradual decline in erectile function over the past couple of years, which can be indicative of hormonal changes. It’s important to have a comprehensive evaluation that includes blood tests to measure testosterone levels, as well as other relevant hormones.
Potential Causes of Hormonal Imbalance
1. Aging: Testosterone levels naturally decline with age, which can lead to erectile dysfunction.
2. Medical Conditions: Conditions such as diabetes, obesity, and hypertension can affect hormone levels and blood flow, contributing to ED.
3. Medications: Some medications can interfere with hormone production or sexual function.
4. Stress and Mental Health: Psychological factors such as anxiety, depression, and stress can also lead to hormonal imbalances and erectile dysfunction.
Treatment Options
1. Hormone Replacement Therapy (HRT): If tests reveal low testosterone levels, your doctor may recommend testosterone replacement therapy. This can be administered through injections, patches, gels, or pellets. It’s essential to discuss the potential benefits and risks with your healthcare provider.
2. Lifestyle Changes: Maintaining a healthy lifestyle can significantly improve erectile function. Regular exercise, a balanced diet, and weight management can enhance blood flow and hormone levels. Reducing stress through mindfulness or therapy can also be beneficial.
3. Medications: In addition to hormone therapy, medications such as phosphodiesterase type 5 inhibitors (e.g., Viagra, Cialis) can help improve blood flow to the penis, facilitating erections.
4. Counseling: If psychological factors are contributing to your erectile dysfunction, counseling or sex therapy may be beneficial. This can help address any underlying anxiety or relationship issues.
5. Alternative Therapies: Some individuals explore herbal supplements or alternative treatments. However, it’s crucial to consult with a healthcare provider before starting any new supplement, as they can interact with medications or have side effects.
Seeking Professional Help
Since you have already consulted with urologists and sexual health specialists without satisfactory results, consider seeking a second opinion from an endocrinologist or a specialist in sexual medicine. They can provide a more comprehensive evaluation of your hormonal health and tailor a treatment plan specific to your needs.
Conclusion
Erectile dysfunction can be a challenging condition, but it is often treatable. Understanding the role of hormones in sexual function is vital, and addressing any imbalances can lead to significant improvements. It’s essential to work closely with healthcare professionals who understand your concerns and can provide personalized care. Remember that you are not alone in this journey, and with the right support and treatment, you can regain your sexual health and confidence.
