What are the symptoms of gonorrhea?

kk, 10~19 year old female. Ask Date: 2007/07/13

Dr. Xu Weikai reply Urology

KK: Hello, the symptoms of gonorrhea include painful urination, frequent urination, urgency to urinate, difficulty urinating, and yellowish, thick discharge from the urethra.
If a bacterial culture of the discharge is performed, it will show an infection with Neisseria gonorrhoeae.
This is a response from Dr.
Hsu Wei-Kai, Director of the Urology Department at Taitung Hospital, Department of Health, Executive Yuan.

More Info

Gonorrhea, caused by the bacterium Neisseria gonorrhoeae, is a sexually transmitted infection (STI) that can affect both men and women. Understanding its symptoms is crucial for early detection and treatment, which can prevent complications and transmission to others.

Symptoms of Gonorrhea
1. In Men:
- Urethritis: The most common symptom in men is a burning sensation during urination. This is often accompanied by a purulent (pus-like) discharge from the penis, which may be yellow, green, or white.

- Swelling and Pain: Some men may experience swelling or pain in the testicles (epididymitis), which can be quite uncomfortable.

- Rectal Symptoms: If the infection spreads to the rectum, men may experience anal itching, discomfort, or discharge.

2. In Women:
- Cervicitis: Women often have milder symptoms, which can include increased vaginal discharge, painful urination, and bleeding between periods or after intercourse. The cervix may become inflamed, leading to pelvic pain.

- Pelvic Inflammatory Disease (PID): If left untreated, gonorrhea can lead to PID, which is an infection of the reproductive organs. Symptoms of PID include severe pelvic pain, fever, and abnormal vaginal discharge.

- Rectal Symptoms: Similar to men, women can also experience rectal symptoms if the infection spreads to that area.

3. Asymptomatic Cases:
- It is important to note that many individuals, especially women, may be asymptomatic. This means they do not exhibit noticeable symptoms, which can lead to delayed diagnosis and treatment. Asymptomatic individuals can still transmit the infection to sexual partners.

Complications of Untreated Gonorrhea
If gonorrhea is not treated, it can lead to serious health issues. In women, untreated gonorrhea can cause PID, which may result in infertility, ectopic pregnancy, and chronic pelvic pain. In men, complications can include epididymitis and, in rare cases, infertility. Additionally, gonorrhea can increase the risk of contracting or transmitting HIV.

Diagnosis and Treatment
Diagnosis of gonorrhea typically involves laboratory testing of urine or swabs from the infected area (urethra, cervix, throat, or rectum). Treatment usually consists of antibiotics. Due to rising antibiotic resistance, the Centers for Disease Control and Prevention (CDC) recommends dual therapy, often involving ceftriaxone and azithromycin.

Prevention
Preventive measures include practicing safe sex by using condoms, having regular STI screenings, and ensuring that sexual partners are tested and treated if necessary. Open communication with partners about sexual health is also vital.

Conclusion
Understanding the symptoms of gonorrhea is essential for timely intervention. If you suspect you may have been exposed to gonorrhea or are experiencing symptoms, it is important to seek medical attention promptly. Early diagnosis and treatment can prevent complications and help control the spread of this infection. Regular screenings and practicing safe sex are key components in managing sexual health and preventing STIs.
