Menstrual issues

Hello Doctor, I have been experiencing irregular menstrual cycles for a while.
I have tried both traditional Chinese medicine and Western medicine for this issue, but as soon as I stop the medication, I experience abnormal bleeding again after a month.
For example, this month (July), my period started on July 3rd and I stopped on July 9th (the bleeding was normal this time), but on July 19th, I had abnormal bleeding again, which was a small amount (not as much as normal menstrual flow) and very dark in color.
I visited a gynecologist (who performed an ultrasound) and the doctor said I have a progesterone deficiency.
He prescribed me pink pills and suggested that I start taking a menstrual regulation medication for three months on the fifth day of my August period (taking it for 21 days each month).

I would like to ask the doctor how to supplement for progesterone deficiency? If I take the menstrual regulation medication for three months, will there be any side effects? If I stop the medication after three months, will my menstrual cycle become irregular again? This situation has happened many times, and the doctor could see from my ultrasound that I have been experiencing abnormal bleeding, which has caused a lot of worry for both me and my mother.
My mother is concerned that this could affect my future fertility (is that true?).
Both Chinese and Western doctors say it is an issue with my constitution, but my mother does not have such problems; why is that? The traditional Chinese medicine doctor advised me to drink Si Wu Decoction after my normal period, but I often get mouth sores, which is very frustrating! Besides taking medication, are there any other methods to improve my condition? Thank you, doctor!

Irregular menstrual periods can be a source of significant concern and discomfort for many women. Understanding the underlying causes, treatment options, and potential lifestyle changes can help manage this condition effectively.

Causes of Irregular Periods
Irregular menstrual cycles can be attributed to various factors, including hormonal imbalances, stress, significant weight changes, polycystic ovary syndrome (PCOS), thyroid disorders, and lifestyle factors such as excessive exercise or poor diet. In your case, the ultrasound indicated a deficiency in progesterone, which is crucial for regulating the menstrual cycle. This deficiency can lead to irregular bleeding and other symptoms.

Treatment Options
1. Hormonal Therapy: As your doctor suggested, taking progesterone supplements can help regulate your menstrual cycle. These medications can help restore the balance of hormones in your body, leading to more regular periods. It’s important to follow your doctor’s instructions regarding dosage and duration.

2. Lifestyle Modifications: Maintaining a healthy lifestyle can significantly impact your menstrual health. Regular exercise, a balanced diet rich in fruits, vegetables, whole grains, and lean proteins, and adequate hydration can help regulate your hormones. Additionally, managing stress through mindfulness practices, yoga, or meditation can also be beneficial.

3. Alternative Therapies: Some women find relief through acupuncture or herbal remedies. However, it’s essential to consult with a healthcare provider before starting any alternative treatments, especially if you are already on medication.

4. Monitoring and Follow-Up: Keeping a menstrual diary can help you and your healthcare provider identify patterns in your cycle, which can be useful for diagnosis and treatment adjustments. Regular follow-ups with your gynecologist are crucial to monitor your condition and make necessary changes to your treatment plan.

Potential Side Effects of Medications
Regarding the pink pills (likely progesterone), common side effects may include mood swings, bloating, breast tenderness, and changes in weight. While many women tolerate these medications well, it’s essential to discuss any side effects you experience with your doctor. If you stop taking the medication after three months, there is a possibility that your irregular periods may return, especially if the underlying hormonal imbalance is not addressed.

Concerns About Fertility
Your mother’s concerns about fertility are valid, as hormonal imbalances can affect ovulation and, subsequently, fertility. However, many women with irregular periods can conceive with appropriate treatment and monitoring. It’s crucial to address any underlying issues early on to improve your chances of a healthy pregnancy in the future.

Additional Recommendations
1. Dietary Adjustments: If you experience mouth sores from certain herbal remedies, it may be worth discussing with your healthcare provider to find alternatives that suit your body better. A diet rich in omega-3 fatty acids, antioxidants, and vitamins can support hormonal balance.

2. Regular Check-Ups: Since you have experienced recurrent abnormal bleeding, it’s important to have regular check-ups to rule out any serious conditions. Your healthcare provider may recommend additional tests if necessary.

3. Support Systems: Engaging with support groups or counseling can help alleviate the emotional burden of dealing with irregular periods. Sharing experiences with others can provide comfort and practical advice.

In conclusion, managing irregular periods often requires a multifaceted approach that includes medical treatment, lifestyle changes, and ongoing communication with healthcare providers. By addressing the hormonal imbalance and making healthy lifestyle choices, you can improve your menstrual health and overall well-being.
