There are small yellowish bumps on the foreskin?

Hello Doctor: I have noticed several small, pale yellow bumps near the glans of my penis.
When I first discovered them, I tried to squeeze them, and some released a yellow substance, similar to dirt from a pimple.
Now they are all small, pale yellow bumps.
After searching online for images, they resemble "genital herpes" (http://www.shute.kh.edu.tw/~healthcare/U20030707002/images/images1/S-%203.jpg), but they do not hurt or itch.
What could these be? Reflecting on my recent lifestyle changes: (1) In June, I soaked in a hot spring once; from July to the first week of September, I often went to bed after 2 AM due to rushing my thesis, sometimes without sleeping at all.
(2) My diet has also changed; in the past two weeks, I have eaten shrimp from a fishing venue four times, usually more than ten shrimp each time, and duck three times.
(3) Last week, I discovered the pale yellow bumps on my foreskin, and this Monday, I noticed oral ulcers.
What puzzles me is that I have not had sexual intercourse for over six months.
Why are there unclean substances appearing on my foreskin? I would like to ask if genital herpes causes pain or itching.
If not, what other conditions could I have? Thank you!

It's really too detailed, but the entire process doesn't seem like a rash infection, and it doesn't look like a "sexually transmitted disease." I suggest you visit a hospital to see a urologist for an examination.

The presence of yellow bumps on the foreskin can be concerning, especially when they resemble conditions like genital herpes. However, based on your description, it sounds more like you may be experiencing a common condition known as pearly penile papules or possibly sebaceous cysts, rather than herpes or another sexually transmitted infection (STI).

Pearly penile papules are small, harmless bumps that typically appear around the corona of the glans (the head of the penis). They are usually skin-colored or slightly yellow and do not cause pain, itching, or discomfort. These bumps are a normal anatomical variation and are not associated with any disease or infection. They are often mistaken for other conditions due to their appearance, but they do not require treatment unless they cause psychological distress.

Sebaceous cysts, on the other hand, are small lumps that can occur when sebaceous glands become blocked. They can sometimes contain a yellowish substance that resembles pus or sebum. Like pearly penile papules, sebaceous cysts are generally harmless and do not require treatment unless they become infected or bothersome.

Regarding your concerns about genital herpes, this condition typically presents with painful blisters or sores, often accompanied by itching or burning sensations. The absence of pain or itching in your case makes herpes less likely. However, it is important to note that herpes can sometimes present with atypical symptoms, so if you have any doubts, it is best to consult a healthcare provider for a proper diagnosis.

Your recent lifestyle changes, including irregular sleep patterns and dietary modifications, could potentially impact your overall health and immune system, but they are unlikely to directly cause the appearance of these bumps. Stress and lack of sleep can weaken the immune system, making you more susceptible to infections, but they do not typically cause benign skin lesions like those you describe.

The presence of canker sores (口內炎) in your mouth could be related to stress, dietary changes, or even a vitamin deficiency, particularly if your diet has changed significantly. Canker sores are common and can occur for various reasons, including stress, hormonal changes, or certain food sensitivities.

In summary, the yellow bumps on your foreskin are likely benign and not indicative of a serious condition, especially given the absence of pain or itching. However, if you are still concerned or if the bumps change in appearance, become painful, or if you develop other symptoms, it is advisable to seek medical attention. A healthcare provider can perform a physical examination and may conduct tests if necessary to rule out any infections or other conditions. Regular check-ups and open communication with your healthcare provider are essential for maintaining sexual health and addressing any concerns you may have.
