Penile issues?
Hello doctor, I would like to ask a question.
I am currently 20 years old and recently (about a month ago) I noticed that if I divide the total length of my penis into three parts, the skin in the middle section has suddenly become a bit dark gray, with small spots that are not very noticeable.
The texture of the skin feels smooth but slightly rough.
Additionally, on the underside of the penis (the side that is closer to the testicles), there are also some small dark spots at the base and a little higher up.
These spots are much darker compared to the less noticeable spots on the front of the penis, but there are only about five of them.
I would like to know if this is a normal phenomenon.
Is it related to normal development, or could it be an infection? (I have been living normally and have not engaged in sexual activity.) Thank you for your response, doctor.

Dr. Xu Weikai reply Urology
A little concern: Hello, based on your description, aside from the need for further evaluation of the black spots, there shouldn't be any major issues; it appears to be normal.
Black spots are difficult to assess through description alone, so it is best to seek medical attention for a more accurate evaluation.
If there are any unclear points, we can further discuss and research together.

Sincerely, Dr.

When it comes to changes in the skin color of the penis, particularly in young men like yourself, there are several factors to consider. First and foremost, it is essential to understand that variations in skin color can occur due to a variety of reasons, including natural pigmentation changes, friction, irritation, or even underlying medical conditions.
In your case, you mentioned that you observed a change in color in the middle section of your penis, which has become somewhat darker and rougher in texture. Additionally, you noted the presence of small dark spots near the base of the penis. While it is common for the skin in this area to undergo changes during puberty and early adulthood, it is crucial to differentiate between normal variations and potential health concerns.
1. Natural Pigmentation Changes: As you grow, hormonal changes can lead to increased melanin production in certain areas of the body, including the genital region. This can result in a darker appearance of the skin. It is not uncommon for young men to notice these changes as they progress through puberty.
2. Friction and Irritation: Activities such as exercise, wearing tight clothing, or even frequent masturbation can cause friction in the genital area, leading to irritation and changes in skin color. If the skin is subjected to repeated friction, it may darken or develop rough patches.
3. Skin Conditions: Certain skin conditions, such as frictional hyperpigmentation or post-inflammatory hyperpigmentation, can also lead to changes in skin color. These conditions are often benign and can resolve on their own with time and proper care.
4. Infections: While you mentioned that you have not engaged in sexual activity, it is still possible to experience infections or skin conditions that can cause changes in color. For example, fungal infections or sexually transmitted infections (STIs) can lead to discoloration or the appearance of spots.
5. Other Factors: Conditions such as diabetes or obesity can also lead to skin changes, including darkening in certain areas. If you have any underlying health issues, it would be wise to consider them as potential contributing factors.
Given your description, it is essential to monitor the changes in your skin. If the discoloration persists, worsens, or is accompanied by other symptoms such as itching, pain, or unusual discharge, it would be prudent to consult a healthcare professional, preferably a dermatologist or a urologist. They can perform a thorough examination and provide an accurate diagnosis.
In the meantime, here are some general recommendations for maintaining skin health in the genital area:
- Hygiene: Keep the area clean and dry. Use mild soap and avoid harsh chemicals that can irritate the skin.
- Moisturization: If the skin feels rough or dry, consider using a gentle moisturizer to help maintain skin hydration.
- Avoid Irritants: Be mindful of clothing choices and avoid tight-fitting garments that can cause friction.
- Sun Protection: Although the genital area is usually covered, if exposed to sunlight, consider using sunscreen to prevent pigmentation changes.
In conclusion, while changes in skin color can be normal during development, it is essential to remain vigilant and seek medical advice if you have concerns. A healthcare professional can provide personalized guidance and ensure that any potential issues are addressed promptly.
