Orthodontics and Teeth Whitening

Hello Dr.
Tsao, I would like to ask some questions regarding orthodontics and teeth whitening.
When I was younger, I underwent orthodontic treatment.
After the treatment, I became lazy and did not continue wearing the retainers.
Now, my lower teeth have shifted a bit.
A few years ago, I consulted a dentist who mentioned that my wisdom teeth were pushing my teeth, so they extracted my left wisdom tooth.
Now, I still have the right one left.
I’m not sure if the right wisdom tooth is causing my lower teeth to tilt slightly to the left.
It seems like my upper and lower teeth are asymmetrical, and sometimes when I speak, my lower jaw appears to lean a little to the left.
Although it’s not very severe, I am quite worried.

I would like to ask if chewing food on one side, either the left or the right, can help alleviate the situation of my lower teeth shifting to the left.
Is there a way to correct this? Some people say that after the age of 20, orthodontic treatment may require surgery.
However, since I had orthodontic treatment when I was younger and my teeth have shifted a bit, can I still undergo correction? How much does orthodontic treatment cost nowadays?
Additionally, regarding teeth whitening, my dentist mentioned that due to my use of tetracycline, my teeth have an undesirable color.
I would like to know if teeth whitening is effective.
Thank you for your help!

Dr. Cao Guoqing reply Dentistry

1.
Please visit an orthodontic specialist clinic for a reevaluation, as it seems that a new orthodontic treatment may be necessary.

2.
The cost varies from person to person, so consider consulting several clinics before making a decision.

3.
Tetracycline can be used for whitening, and it typically takes over a year to be effective.

When it comes to orthodontics and teeth whitening, addressing both misalignment and discoloration can be a multifaceted process. Based on your concerns, let's break down the issues and explore potential solutions.

Misalignment of Teeth
From your description, it seems that your lower teeth have shifted due to the absence of a retainer after your initial orthodontic treatment. This is a common issue; without proper retention, teeth can gradually move back toward their original positions or shift due to the pressure from wisdom teeth. In your case, the remaining wisdom tooth may be contributing to the misalignment of your lower teeth.

1. Chewing Habits: While it’s understandable to want to mitigate the shifting by favoring one side for chewing, this is not a long-term solution. Chewing predominantly on one side can lead to uneven wear and potential jaw issues. Instead, it’s advisable to consult with an orthodontist who can assess your situation and recommend appropriate treatment.

2. Orthodontic Treatment: Given that you have previously undergone orthodontic treatment, it is indeed possible to have a second round of orthodontics. Many adults seek orthodontic care to correct shifts that occur over time. Depending on the severity of the misalignment, options may include traditional braces, clear aligners (like Invisalign), or even surgical options if necessary. The cost of orthodontic treatment can vary widely based on the complexity of the case and the type of treatment chosen, typically ranging from $3,000 to $8,000 in the U.S.

3. Surgical Options: While surgery is sometimes necessary for severe cases of misalignment, many patients can achieve satisfactory results with braces or aligners alone. Your orthodontist will be able to provide a tailored treatment plan based on a thorough examination.

Teeth Whitening
Regarding the discoloration of your teeth, particularly due to tetracycline use, this can be more challenging to treat. Tetracycline staining often results in a grayish or brownish hue that is resistant to traditional whitening methods.

1. Effectiveness of Whitening: While some patients with tetracycline stains may see improvement with professional whitening treatments, results can vary significantly. In many cases, whitening may not fully resolve the discoloration. Your dentist may recommend alternative treatments, such as porcelain veneers or crowns, which can effectively cover the stained teeth and improve aesthetics.

2. Professional vs. At-Home Whitening: Professional whitening treatments tend to be more effective than over-the-counter options, especially for stubborn stains. However, it’s essential to have a consultation with your dentist to discuss the best approach for your specific situation.

3. Long-Term Care: After any whitening treatment, maintaining good oral hygiene and regular dental check-ups will help preserve the results. Additionally, avoiding foods and drinks that can stain teeth, such as coffee, tea, and red wine, can be beneficial.

Conclusion
In summary, addressing both misalignment and discoloration of teeth requires a comprehensive approach. Consulting with an orthodontist will provide clarity on the best course of action for your misaligned teeth, while a dentist can guide you on effective whitening options tailored to your specific needs. Remember, it’s never too late to seek treatment, and advancements in dental care can offer solutions that were not available in the past. Regular dental visits and open communication with your dental care providers will be key in achieving the smile you desire.
