Does this happen occasionally?
Hello, Doctor Hsu.
Why do I occasionally feel the sensation of ejaculation when my penis is in a flaccid state? It feels like semen is about to be expelled from the urethra.
Additionally, sometimes when I have an erection without any masturbation, my penis feels very engorged, almost to the point of ejaculation.
At times, even just having an erection can trigger this sensation of impending ejaculation.
What could be the reason for this? If I experience this, would you recommend masturbation to ejaculation? I am quite concerned about potentially ejaculating involuntarily! I rarely masturbate! Could this be due to infrequent masturbation?

It's normal to have such feelings.
Whether or not to ejaculate is entirely up to you.
There is no definitive statement that ejaculation is better or worse; it completely depends on individual needs.
Additionally, the urge to ejaculate when experiencing an erection is also quite normal, especially in individuals with no sexual experience.
If you have any further questions or need more clarification, feel free to ask for further discussion.

Understanding occasional ejaculation sensations in men can be quite complex, as it involves a combination of physiological and psychological factors. The sensations you describe—feeling as if ejaculation is imminent even without direct stimulation—can be attributed to several potential causes.
Firstly, it’s important to recognize that the male reproductive system is highly sensitive and responsive to various stimuli, both physical and psychological. The sensations you are experiencing could be related to heightened sensitivity in the genital area, which can occur for a variety of reasons, including anxiety, arousal, or even the anticipation of sexual activity. This heightened sensitivity can lead to sensations that mimic the feeling of ejaculation, even in the absence of direct stimulation.
Another factor to consider is the role of the prostate gland. The prostate is responsible for producing a significant portion of the fluid that makes up semen. In some cases, if the prostate is inflamed or irritated—such as in cases of prostatitis—it can lead to unusual sensations in the genital area, including the feeling of impending ejaculation. Chronic pelvic pain syndrome, which is often associated with prostatitis, can also contribute to these sensations.
Additionally, the phenomenon you describe may be linked to the concept of "blue balls," a colloquial term that refers to the discomfort some men experience when they are aroused for an extended period without ejaculation. This can create a feeling of fullness or pressure in the testicles and can sometimes be accompanied by sensations that feel similar to ejaculation.
Regarding your question about whether infrequent ejaculation could be contributing to these sensations, it is possible. Regular ejaculation can help relieve built-up pressure and may reduce the frequency of such sensations. However, it’s essential to approach this with balance; excessive or compulsive masturbation can lead to its own set of issues, including desensitization or performance anxiety.
If you find that these sensations are causing you distress or interfering with your daily life, it may be beneficial to consult with a healthcare professional, particularly a urologist. They can provide a thorough examination and rule out any underlying medical conditions, such as prostatitis or other pelvic floor disorders.
In terms of solutions, here are a few strategies you might consider:
1. Regular Ejaculation: If you feel comfortable, increasing the frequency of ejaculation—whether through masturbation or sexual activity—might help alleviate some of the sensations you are experiencing.
2. Mindfulness and Relaxation Techniques: Stress and anxiety can exacerbate sensitivity and discomfort. Practicing mindfulness, deep breathing, or other relaxation techniques may help you manage these feelings.
3. Pelvic Floor Exercises: Strengthening the pelvic floor muscles through exercises like Kegels can improve control over ejaculation and may help with the sensations you are experiencing.
4. Consultation with a Specialist: If the sensations persist or worsen, seeking advice from a urologist or a sexual health specialist can provide you with tailored guidance and treatment options.
In conclusion, while occasional sensations of impending ejaculation can be a normal part of male physiology, persistent or distressing symptoms warrant further investigation. Understanding your body and its responses is key, and professional guidance can help you navigate any concerns you may have.
