Sexual dysfunction issues
I have a question for Dr.
Zhang.
About two years ago, I injured the right side of my abdomen while playing badminton, which was very painful.
I consulted an orthopedic doctor, and I recovered in about a week.
However, at that time, I noticed that my usual morning erections suddenly disappeared, and I lost interest in sex, even with my girlfriend.
I experienced erectile dysfunction, feeling aroused but unable to perform.
(Prior to this, everything was normal for me.) During that time, I had tests done, and my prolactin levels were within the normal range; the doctor said I was overthinking it.
Recently, after two years, I went for a check-up and my testosterone level was 755 ng/dL, creatinine was 1 mg/dL, and both GOT and GPT were normal.
My fasting blood sugar was 102.
I maintain a normal lifestyle, my mental and physical health are fine, and I have no bad habits or any diseases, including hypertension.
Dr.
Zhang, what do you think is going on with me? Could this be an issue related to the autonomic nervous system? This situation has been troubling me.
Do I need further treatment or tests? Is there any way to improve this condition? Thank you for your detailed response, and I wish you all the best.

Erectile dysfunction (ED) is a common issue that can affect men of all ages, but it often becomes more prevalent with age. Your situation, particularly following a traumatic incident, raises several important considerations regarding the potential causes and solutions for your erectile dysfunction.
Possible Causes of Erectile Dysfunction
1. Psychological Factors: Given that your erectile dysfunction began after a traumatic injury, psychological factors such as anxiety, stress, or depression may play a significant role. The mind-body connection is powerful, and psychological distress can lead to physical symptoms, including ED.
2. Neurological Issues: The trauma you experienced could have affected the nerves involved in the erectile process. The autonomic nervous system, which controls involuntary bodily functions, plays a crucial role in achieving and maintaining an erection. If there was any nerve damage or disruption due to the injury, it could lead to erectile dysfunction.
3. Hormonal Imbalances: Although your testosterone levels are within the normal range, it’s important to consider other hormonal factors. For instance, elevated levels of prolactin, thyroid hormones, or imbalances in estrogen can also contribute to ED. You mentioned that your prolactin levels were normal, which is a positive sign, but further hormonal evaluations might be warranted.
4. Vascular Health: Erectile function is heavily dependent on adequate blood flow to the penis. Conditions that affect vascular health, such as diabetes, high blood pressure, or high cholesterol, can lead to ED. Your blood sugar levels are slightly elevated (102 mg/dL), which may indicate prediabetes, a condition that can affect vascular health over time.
5. Lifestyle Factors: Although you mentioned having a healthy lifestyle, factors such as diet, exercise, and sleep can significantly impact erectile function. Regular physical activity, a balanced diet, and adequate sleep are essential for maintaining hormonal balance and overall health.
Solutions and Recommendations
1. Psychological Counseling: If psychological factors are suspected, consider speaking with a mental health professional. Therapy can help address anxiety, stress, or any underlying emotional issues that may be contributing to your ED.
2. Further Medical Evaluation: It may be beneficial to undergo a more comprehensive evaluation, including:
- Hormonal Testing: Check for other hormones like thyroid hormones and estrogen levels.
- Vascular Assessment: A Doppler ultrasound can assess blood flow to the penis, helping to identify any vascular issues.
- Neurological Examination: A consultation with a neurologist may be warranted to evaluate any potential nerve damage from your injury.
3. Lifestyle Modifications: Continue to maintain a healthy lifestyle. Regular exercise can improve blood flow and reduce stress. Additionally, consider dietary changes that promote vascular health, such as increasing fruits, vegetables, whole grains, and healthy fats while reducing processed foods and sugars.
4. Medications: If lifestyle changes and psychological counseling do not yield improvements, medications such as phosphodiesterase type 5 inhibitors (e.g., Viagra, Cialis) may be considered. These medications can enhance blood flow to the penis and improve erectile function.
5. Follow-Up: Regular follow-up with your healthcare provider is essential to monitor your condition and adjust treatment as necessary. Open communication about your symptoms and any changes you experience will help guide your management plan.
In conclusion, while your situation is understandably concerning, there are multiple avenues to explore for diagnosis and treatment. Addressing both the physical and psychological aspects of erectile dysfunction will provide the best chance for improvement. Don’t hesitate to reach out to your healthcare provider for further evaluation and support.
