I don't understand~~!
Hello, Dr.
Hsu.
My question is, while others can achieve a full erection through fantasy, why am I unable to do so like an average person? I exercise regularly and have normal sleep patterns.
Additionally, when I am intimate with my girlfriend, I need her to help me achieve an erection before we can proceed.
Are there any methods to improve this? I have previously seen a doctor who mentioned it might be a psychological issue, but I feel mentally fine during intimacy or when fantasizing.
The doctor also suggested I take Viagra, which I declined because I am only 25 years old.
If I start taking it now, how long will I have to continue? Please help me with these questions.
Thank you.
Xingxing~, 20~29 year old female. Ask Date: 2008/01/16

Gorilla: Hello, many people can indeed reach the state of erection or even orgasm through fantasy.
However, not everyone has this ability.
Some individuals, especially women, have a very vivid imagination and can intricately create a sensory experience through their thoughts; on the other hand, some people may be more reserved or lack imagination, making it difficult to achieve an erection.
Some individuals need to see visual stimuli (such as adult films) to become aroused, while others require direct stimulation, such as touch.
In other words, as long as the appropriate method to achieve an erection is found and both partners can cooperate, it becomes a matter of habit.
Additionally, psychological factors may also play a role, such as subconscious beliefs instilled from a young age that premarital sex is wrong, or fears of pregnancy, among others.
These can contribute to psychological barriers to erection.
Viagra, in fact, does not need to be viewed with such resistance or hostility.
Generally, when we face patients, especially those who suspect they have psychological barriers or have experienced several failed attempts at intercourse and have completely lost confidence, using Viagra for one or two instances can help achieve a better state of erection.
Most patients, particularly those with erectile dysfunction due to psychological reasons, can regain their confidence through a few effective treatment sessions, thereby overcoming their barriers.
In follow-up assessments, most patients find that they no longer need to rely on medication and even find it amusing to think that they once experienced erectile dysfunction.
If there are any details that need further clarification, please feel free to ask again, and we can discuss and research together.

Erectile dysfunction (ED) is a common issue that can affect men of all ages, including those as young as 25. It’s understandable to feel concerned about your ability to achieve and maintain an erection, especially when you notice that you require additional stimulation from your partner to initiate sexual activity. While it’s good to hear that you maintain a healthy lifestyle with regular exercise and adequate sleep, it’s important to consider that ED can be influenced by a variety of factors, including psychological, emotional, and physical components.
From what you've described, it seems that your doctor has suggested that your erectile issues may be psychological in nature. This can be a common diagnosis, especially in younger men who may not have underlying physical health issues. Psychological factors can include anxiety, stress, or even performance anxiety, which can create a cycle of worry that makes it even harder to achieve an erection. Even if you feel mentally prepared, subconscious fears or anxieties can still play a significant role in sexual performance.
Here are some strategies that may help you improve your situation:
1. Communication with Your Partner: Openly discussing your feelings and experiences with your girlfriend can help alleviate some of the pressure you may feel during intimate moments. This can foster a supportive environment where both of you can explore what feels good without the added stress of performance.
2. Relaxation Techniques: Engaging in relaxation techniques such as deep breathing, meditation, or yoga can help reduce anxiety levels. Lowering your overall stress can positively impact your sexual performance.
3. Counseling or Therapy: If you suspect that psychological factors are at play, consider seeking the help of a therapist or counselor who specializes in sexual health. Cognitive-behavioral therapy (CBT) can be particularly effective in addressing performance anxiety and other psychological barriers to sexual function.
4. Gradual Exposure: Instead of focusing solely on penetration, try to explore other forms of intimacy with your partner. This can help reduce the pressure associated with achieving an erection and allow you to enjoy the experience more fully.
5. Lifestyle Adjustments: While you mentioned that you exercise regularly, ensure that you are also maintaining a balanced diet, managing stress, and avoiding excessive alcohol or tobacco use, as these can all contribute to erectile issues.
6. Medical Consultation: If you continue to experience difficulties, it may be worthwhile to revisit your healthcare provider. While you’ve expressed reluctance to use medications like Viagra (sildenafil), it’s important to have an open dialogue about your concerns. Medications can be a temporary solution while you explore other avenues, and many men find that they can eventually manage their ED without relying on medication long-term.
7. Education and Understanding: Sometimes, understanding the physiological aspects of arousal and erection can help demystify the process. Knowing that it’s normal for men to experience fluctuations in their ability to achieve an erection can help ease some of the pressure.
In conclusion, while it’s understandable to want to avoid medication at a young age, it’s essential to address the underlying issues contributing to your erectile dysfunction. By exploring both psychological and physical avenues, you can work towards a solution that feels right for you. Remember, seeking help is a sign of strength, and addressing these issues can lead to a more fulfilling sexual experience in the long run.
