After taking RU486..?

Hello doctor, I would like to ask you some questions.
On March 4th, I experienced some light bleeding and thought my period had started, but it only lasted for 3 days and the flow was very minimal.
So, I went to see a gynecologist, and at that time, they told me I was one month pregnant, but the embryo was still very small and couldn't be seen clearly.
On March 11th, I asked a friend to help me obtain RU486, and I took 3 pills on the first day.
Within 4 hours, I started bleeding.
After 36 hours, I began taking medication to induce uterine contractions daily for 6 days.
During the medication period, I did not experience any significant discomfort.
However, two days ago, the bleeding changed to brown discharge, and as of today, March 22nd, the brown discharge has stopped completely, and I feel no discomfort.
I would like to ask the doctor, since the embryo was not visible before but I have taken the medication and experienced bleeding and blood clots, is there still a possibility of an ectopic pregnancy?

Dr. Wu Zinan reply Obstetrics and Gynecology

Hello, Hsiao-Ting: 1.
Please take a pregnancy test to confirm whether you are pregnant.
2.
If pregnancy is confirmed but no gestational sac is seen on ultrasound, you should not take medication to avoid the risk of an ectopic pregnancy without knowing.
3.
You can have two blood tests to determine the B-HCG levels to confirm whether you are pregnant or have had a miscarriage.

When considering the use of RU486 (mifepristone) and the potential for complications such as ectopic pregnancy, it's essential to understand the mechanisms of the drug and the implications of its use. RU486 is primarily used for medical abortion and works by blocking progesterone, a hormone necessary for maintaining pregnancy. This leads to the detachment of the embryo from the uterine wall and, subsequently, the expulsion of the pregnancy tissue.

In your case, you mentioned experiencing some bleeding and then taking RU486, followed by a series of medications to induce uterine contractions. The fact that you had minimal discomfort and that the bleeding transitioned to brown discharge suggests that your body is responding to the medication. However, the concern about ectopic pregnancy arises when there is uncertainty about the location of the embryo, especially if it was not visible on ultrasound.

Ectopic pregnancy occurs when a fertilized egg implants outside the uterus, most commonly in the fallopian tubes. This condition can be life-threatening if not diagnosed and treated promptly. Symptoms of ectopic pregnancy may include sharp abdominal pain, vaginal bleeding, and signs of internal bleeding, such as dizziness or fainting. Since you have not reported these symptoms and have experienced a decrease in discharge, the likelihood of an ectopic pregnancy may be lower, but it is not entirely ruled out.

The absence of visible embryonic tissue on ultrasound at the time of your initial examination could indicate a very early pregnancy or a potential issue with the pregnancy itself. After taking RU486, if the pregnancy is not expelled completely, there could still be residual tissue, which might complicate the situation. It's crucial to follow up with your healthcare provider for further evaluation. They may recommend additional ultrasounds or blood tests to measure the levels of human chorionic gonadotropin (hCG), which can help determine whether the pregnancy is progressing normally or if there are complications such as an ectopic pregnancy.

In summary, while your current symptoms may not strongly indicate an ectopic pregnancy, it is vital to maintain close communication with your healthcare provider. They can provide the necessary monitoring and interventions to ensure your health and safety. If you experience any new or worsening symptoms, such as severe abdominal pain or heavy bleeding, seek medical attention immediately. Remember, early detection and treatment are key in managing any potential complications related to pregnancy.
