The skin is very itchy?

Hello, Dr.
Hong.
The skin in the small area between my scrotum and the top of my anus has become red and itchy since this past Monday.
When I shower, the water hitting that area causes a stinging sensation.
I went to see a doctor, but they didn't specify the cause; they just advised me to keep it clean and prescribed medication, mentioning that it contains steroids and should only be used when absolutely necessary.
I would like to ask: 1.
Is this a fungal infection or something else? 2.
What additional cleaning or care should I pay attention to? Thank you for your response.

Dr. Hong Zhengsheng reply Dermatology

Hello, Phile: Skin tinea, eczema, localized keratosis abnormalities, localized small nerve abnormalities related to the skin, and localized infectious skin conditions are all possible causes.
I apologize for not being able to provide a diagnosis based solely on your description.
My best advice is to consult a dermatologist you trust.
I recommend that when you notice the symptoms described, you should promptly visit a public hospital in your city or county and consult with a dermatology specialist.

It sounds like you're experiencing a challenging situation with your skin, particularly in the sensitive area between your scrotum and anus. The symptoms you describe—redness, itching, and a stinging sensation when water touches the area—can indeed be indicative of a few different conditions, including fungal infections, irritant dermatitis, or even intertrigo, which is inflammation that occurs in skin folds.

1. Possible Causes:
- Fungal Infection: One common cause of itching and redness in the genital area is a fungal infection, such as tinea cruris (commonly known as jock itch). This condition is often characterized by a red, itchy rash that can spread to the inner thighs and buttocks. Fungal infections thrive in warm, moist environments, making the groin area particularly susceptible.

- Irritant Dermatitis: This can occur due to friction, moisture, or irritants such as soaps, detergents, or even sweat. If you’ve recently changed any products you use in that area, it could be a reaction to those.

- Intertrigo: This is another possibility, especially if the area is warm and moist. It occurs when skin rubs against skin, leading to irritation and inflammation.

2. Care and Hygiene:
- Keep the Area Dry: Moisture can exacerbate fungal infections and irritant dermatitis. After bathing, ensure the area is thoroughly dried. You might consider using a hairdryer on a low setting to help with this.

- Use Gentle Cleansers: Avoid harsh soaps or products that can irritate the skin. Opt for mild, fragrance-free cleansers.

- Wear Loose-fitting Clothing: Tight clothing can cause friction and trap moisture. Loose-fitting underwear made from breathable fabrics like cotton can help keep the area dry.

- Avoid Scratching: While it may be tempting, scratching can worsen the irritation and lead to secondary infections.

3. Topical Treatments:
- The steroid cream prescribed by your doctor can help reduce inflammation and itching, but it should be used sparingly and only when necessary, as overuse can lead to skin thinning and other side effects.

- If a fungal infection is suspected, antifungal creams may be more appropriate. These can be over-the-counter or prescription-based, depending on the severity.

4. When to Seek Further Medical Attention:
- If the symptoms persist despite following good hygiene practices and using the prescribed treatments, or if they worsen, it’s important to return to your healthcare provider. They may need to perform a skin scraping or culture to determine the exact cause of your symptoms and provide a more targeted treatment.

5. Preventive Measures:
- To prevent future occurrences, maintain good hygiene, keep the area dry, and consider using antifungal powder if you are prone to fungal infections, especially during hot weather or after exercising.

In summary, while your symptoms could be due to a fungal infection, irritant dermatitis, or intertrigo, proper hygiene and care are crucial in managing and preventing these conditions. If the situation does not improve, further evaluation by a dermatologist or healthcare provider is recommended for a definitive diagnosis and treatment plan.
