Is it normal to experience the following symptoms after abruptly stopping medication?

Hello: My father is 78 years old and regularly undergoes health check-ups.
Last year, he was diagnosed with rheumatoid arthritis and was prescribed Donison (5) and Acetal (500mg), which he is currently taking as directed.
He has also undergone blood tests, urinalysis, and ultrasound at Tzu Chi Hospital, all of which returned normal results.
He thought he could stop taking the medication since he wasn't experiencing any pain, but once he stopped, he began to experience widespread soreness, chills, weakness, involuntary hand tremors, cold sweats, and even fainting episodes.
Is this normal? What are the functions of these two medications?

Benben, 30~39 year old female. Ask Date: 2008/04/16

Dr. Lin Xiaoyi reply Orthopedics

Hello: Donison is a type of steroid - prednisolone, used to treat inflammatory conditions.
Acetal is acetaminophen, used for pain relief and fever reduction.
Based on your description, these symptoms may be due to the sudden discontinuation of steroids.
Long-term use of steroids can suppress the body's ability to produce its own steroids, and if the medication is stopped abruptly, the body's ability to synthesize steroids may not have recovered, leading to these symptoms.
I have two recommendations: 1.
Find a trusted rheumatologist and have regular consultations to assess whether it is indeed a case of rheumatoid arthritis.
2.
If it is confirmed to be rheumatoid arthritis, immediate treatment is necessary, as untreated cases can lead to severe joint deformities and disabilities.
Therefore, early-stage treatment often requires the use of immunomodulatory medications.
3.
When discontinuing long-term steroid use, it is essential to taper the dosage gradually to avoid severe symptoms; please consult your physician on how to reduce the dosage.
Wishing you good health, Dr.

More Info

It is not uncommon for individuals to experience symptoms after discontinuing arthritis medication, particularly if they have been on corticosteroids like Donison (prednisolone) for an extended period. Your father's symptoms of widespread pain, cold sweats, weakness, tremors, and even fainting can be indicative of withdrawal effects, especially considering the medication he was taking.

Understanding the Medications
1. Donison (Prednisolone): This is a corticosteroid used to reduce inflammation and suppress the immune system. It is commonly prescribed for conditions like rheumatoid arthritis to help manage symptoms and prevent flare-ups. Long-term use of corticosteroids can lead to the body becoming reliant on the medication for cortisol production. When the medication is suddenly stopped, the body may struggle to produce adequate amounts of cortisol on its own, leading to withdrawal symptoms.

2. Acetal (Acetaminophen): This medication is primarily used for pain relief and to reduce fever. It does not have the same withdrawal effects as corticosteroids, but it is important for managing pain associated with arthritis.

Symptoms After Stopping Medication
The symptoms your father is experiencing after stopping Donison are concerning and may suggest adrenal insufficiency due to the abrupt cessation of corticosteroid therapy. When corticosteroids are taken for a long time, they can suppress the hypothalamic-pituitary-adrenal (HPA) axis, which is responsible for cortisol production. If the medication is stopped suddenly, the body may not be able to produce enough cortisol to meet physiological needs, leading to symptoms such as:
- Fatigue and Weakness: The body may feel weak and fatigued due to insufficient cortisol.

- Muscle Pain: Withdrawal can lead to generalized muscle pain or discomfort.

- Cold Sweats and Tremors: These can be signs of adrenal crisis or withdrawal symptoms.

- Fainting: This can occur due to low blood pressure or inadequate blood flow, which may be exacerbated by the lack of cortisol.

Recommendations
1. Consult a Rheumatologist: It is crucial to consult with a rheumatologist or a healthcare provider who specializes in autoimmune diseases. They can evaluate your father's condition and determine whether he should resume medication or if a tapering schedule is necessary.

2. Gradual Tapering: If your father has been on corticosteroids for a long time, it is generally recommended to taper the dosage gradually rather than stopping abruptly. This allows the body to adjust and start producing cortisol naturally again.

3. Monitoring Symptoms: Keep a close eye on his symptoms. If they worsen or if he experiences severe symptoms such as fainting or severe weakness, seek immediate medical attention.

4. Consider Alternative Treatments: If rheumatoid arthritis is confirmed, there may be alternative treatments or medications that can help manage his symptoms without the risks associated with sudden withdrawal from corticosteroids.

Conclusion
In summary, while it is not uncommon to experience symptoms after stopping arthritis medication, the severity of your father's symptoms suggests that he may be experiencing withdrawal effects from the corticosteroid. It is essential to consult with a healthcare provider to address these symptoms appropriately and to discuss a safe plan for managing his rheumatoid arthritis.
