Small red rashes appear on the glans?
Hello Dr.
Hsu, I apologize for not clearly stating my age in my previous question, which may have led to some misunderstanding.
I am a 22-year-old male.
Recently, I have been experiencing red spots on the glans that resemble small rashes.
The area of the rash seems to be spreading; when the left side of the glans improves, the right side starts to develop a rash.
Additionally, the foreskin that comes into contact with the glans has also started showing rashes.
These rashes appear quickly and usually resolve on their own within 2-3 days, but the areas where the rashes occurred remain slightly red and feel a bit wrinkled afterward.
During this time, I consulted a doctor who suggested that it might be due to a bacterial infection and phimosis.
The doctor mentioned that as long as there is no pain, no open sores, and no significant issues, there is no need for medication, so I was not prescribed anything.
I would like to ask you a few questions:
1.
Given that the rashes have recurred almost every week for the past 2-3 weeks, what could be the underlying cause? (I have been maintaining cleanliness and wash the glans with warm water daily during showers.)
2.
I have seen that you recommend applying baby oil on the glans after bathing to prevent infection.
In my case, is it advisable to use it? If so, does it matter which brand of baby oil I use, and how much should I apply?
3.
The doctor I visited previously suggested that I consider circumcision, as my condition might be due to phimosis.
In my situation, should I proceed with the circumcision? Will it truly lead to a complete resolution of my issues?
Thank you for patiently reading my questions.

Dr. Xu Weikai reply Urology
A-Zhi: Hello, for cleaning the glans, it is generally recommended to use cold water without any cleaning agents or hot water.
Baby oil is usually acceptable; there is no specific brand required, just apply it evenly.
If you feel dryness, you can apply a thicker layer.
Additionally, if possible, when a rash appears, it's best to rinse with clean water after each urination.
After circumcision, if proper care is not taken, the risk of inflammation will not decrease; the key is to focus on personal hygiene and cleaning methods.
If there are any unclear points, feel free to ask again for further discussion and research.

Understanding red bumps on the glans can be concerning, especially for a young male like yourself. Based on your description, it seems you are experiencing recurrent red bumps or rashes on the glans (the head of the penis), which can be attributed to several potential causes. Let's address your questions one by one.
1. Possible Causes of Recurring Red Bumps: The recurrent nature of your symptoms suggests that there may be an underlying issue that needs to be addressed. Common causes of red bumps on the glans include:
- Bacterial Infections: As your doctor mentioned, bacterial infections can lead to inflammation and the appearance of red bumps. These infections can be exacerbated by factors such as poor hygiene, irritation from soaps or lotions, or even friction from sexual activity.
- Fungal Infections: Conditions like candidiasis (yeast infection) can also cause similar symptoms, particularly if you are in a warm, moist environment.
- Irritation or Allergic Reactions: Sometimes, the skin can react to certain products (like soaps, lotions, or even condoms) leading to dermatitis, which can manifest as red bumps.
- Phimosis: If you have a tight foreskin (phimosis), it can lead to irritation and inflammation of the glans, contributing to your symptoms.
2. Use of Baby Oil: Using baby oil after bathing can help to moisturize the skin and may provide a barrier against irritants. However, it’s essential to ensure that the product is hypoallergenic and free from fragrances or additives that could cause irritation. A small amount should suffice; you don't need to apply a thick layer. Just a light coating to keep the area moisturized should be adequate. Brands like Johnson's Baby Oil are commonly used, but always check for any allergic reactions when trying a new product.
3. Circumcision Consideration: Your doctor’s suggestion to consider circumcision is not uncommon for individuals with recurrent issues related to phimosis or persistent infections. Circumcision can help reduce the risk of infections and make hygiene easier. However, it is essential to weigh the benefits against the risks and consider your personal preferences. Many individuals find relief from recurrent infections after circumcision, but it is not a guaranteed solution for everyone. It would be best to discuss this option further with a urologist who can provide a thorough evaluation and help you make an informed decision.
In addition to these points, maintaining good hygiene is crucial. Ensure that you clean the area gently but thoroughly, avoiding harsh soaps that can irritate the skin. If you notice any additional symptoms such as pain, discharge, or persistent redness, it’s essential to seek medical attention promptly.
Lastly, if your symptoms persist despite good hygiene and the use of moisturizers, or if you experience any new symptoms, it would be wise to follow up with a healthcare provider. They may recommend further testing or a different treatment approach, such as topical antifungal or antibacterial medications, depending on the underlying cause of your symptoms.
In summary, while the red bumps on your glans may not be a cause for immediate concern, understanding their cause and addressing any underlying issues is essential for your health and comfort. Regular follow-ups with your healthcare provider will help ensure that you receive the appropriate care and guidance.
