Frequent urination?

I have been experiencing frequent urination issues, and there was one severe episode where I had blood in my urine and intense pain.
After that incident, this symptom has been recurring easily.
Recently, it has started again; I find myself needing to urinate every time there is a commercial on TV.
I also wake up at night to use the bathroom, which disrupts my sleep quality.
This morning, I woke up with a tight, aching pain in my abdomen, especially on the right side, and it's quite uncomfortable.
What should I do?

Dr. Xu Weikai reply Urology

Hello, based on your description, it seems that you may have had an acute bladder infection previously, which has resulted in some injury to the bladder.
This means there could be minor trauma that makes it easier for bacteria to get trapped.
I recommend drinking plenty of water.
If you are concerned about it affecting your sleep, try to avoid drinking water one hour before bedtime.
Generally, it is advised to consume about 2500-3000 cc of water daily.
If possible, moderately consuming cranberry juice or products can help reduce the chances of recurrence.
For instance, when you are experiencing pain and symptoms, you can drink 500 cc of cranberry juice (or an equivalent amount of products) daily.
Once the symptoms subside, you can reduce it to 300 cc daily for about two weeks as a maintenance measure, which should effectively address your issue.
Additionally, developing a habit of urinating when you feel the urge and not allowing the bladder to become overly full is fundamental to resolving the problem.
If you have any further questions or need more details, feel free to ask, and we can discuss it further.
Sincerely, Dr.
Hsu Wei-Kai, Director of Urology, Taitung Hospital, Department of Health, Executive Yuan.

Frequent urination, blood in urine, and pain can be indicative of various urological conditions that require careful evaluation and management. Your symptoms, including the urgency to urinate frequently, the presence of blood, and abdominal discomfort, are concerning and warrant immediate medical attention.

Firstly, frequent urination, especially when accompanied by pain and blood, can be a sign of urinary tract infections (UTIs), bladder infections, or even more serious conditions such as interstitial cystitis or bladder cancer. The fact that you have experienced these symptoms repeatedly suggests that there may be an underlying issue that needs to be addressed.

The presence of blood in your urine (hematuria) is particularly alarming. It can arise from several sources, including infections, kidney stones, or trauma to the urinary tract. In some cases, it may indicate more serious conditions such as tumors in the bladder or kidneys. Therefore, it is crucial to undergo a thorough evaluation, which may include urinalysis, imaging studies (like ultrasound or CT scans), and possibly a cystoscopy, where a small camera is inserted into the bladder to visualize any abnormalities.

The abdominal pain you are experiencing, especially if localized to one side, could be related to several factors. It might be associated with kidney stones, which can cause severe pain and lead to hematuria. Alternatively, it could be related to muscle strain, gastrointestinal issues, or even referred pain from the urinary tract. Given the complexity of your symptoms, it is essential to consult a urologist who can provide a comprehensive assessment.

In the meantime, here are some general recommendations that may help alleviate your symptoms:
1. Stay Hydrated: Drinking plenty of water can help flush out your urinary system and may reduce the concentration of irritants in your urine.

2. Avoid Irritants: Certain foods and beverages, such as caffeine, alcohol, spicy foods, and artificial sweeteners, can irritate the bladder and exacerbate symptoms. Consider reducing or eliminating these from your diet.

3. Pain Management: Over-the-counter pain relievers, such as ibuprofen or acetaminophen, may help alleviate discomfort. However, consult your doctor before taking any medication, especially if you have underlying health conditions or are taking other medications.

4. Monitor Symptoms: Keep a diary of your symptoms, noting when they occur, their severity, and any potential triggers. This information can be invaluable for your healthcare provider in diagnosing your condition.

5. Seek Immediate Care: If you experience severe pain, fever, chills, or worsening symptoms, seek medical attention promptly. These could be signs of a more serious condition that requires urgent treatment.

In conclusion, while your symptoms are distressing, they are not uncommon in urological practice. The key to managing your condition lies in obtaining a proper diagnosis and following through with the recommended treatment plan. Please make an appointment with a urologist as soon as possible to discuss your symptoms in detail and undergo the necessary evaluations. Early intervention can significantly improve your quality of life and address any underlying issues effectively.
