Dizziness issues?

Hello Doctor: When I walk, I feel lightheaded, and I also sense that the floor is floating.
I went to the hospital for tests including posturography, hearing tests, eye movement tests, and temperature difference tests.
During the temperature difference test, it seemed that there was an issue with blood circulation in my left ear.
Therefore, I started taking 80 mg of Cerenin daily.
After starting the medication, my symptoms did improve significantly, but they have not completely resolved; I still feel the ground floating and my body feels lightheaded.
I would like to ask if I need to take Cerenin for a longer period (such as three months or even six months) for a potential complete resolution of my condition, or if the current level of improvement is the maximum effect of Cerenin.
I noticed improvement just three hours after taking the medication for the first time, but after continuing for about a month, there hasn't been any significant further progress.

Dr. Gao Chunde reply Neurology

CERENIN is designed to improve your inner ear circulation; however, any established damage must rely on the body's own repair and adaptation mechanisms.
At this point, the effectiveness of CERENIN is less related to your current residual symptoms.
Identifying and preventing risk factors for circulatory disturbances, along with maintaining a regular lifestyle to enhance physical strength and nerve repair, is the fundamental approach.

Dizziness can be a perplexing symptom, often leading patients to seek answers regarding its underlying causes and appropriate treatments. In your case, it appears that you have undergone a series of diagnostic tests, including balance assessments and hearing evaluations, which have indicated potential issues with blood circulation in your left ear. This finding, along with your symptoms of feeling lightheaded and experiencing a floating sensation, suggests that your dizziness may be related to vestibular dysfunction or possibly a condition affecting the inner ear.

You mentioned that you have been prescribed Cerenin (betahistine), which is commonly used to treat conditions like Meniere's disease and other vestibular disorders. Betahistine works by improving blood flow to the inner ear, which can help alleviate symptoms of dizziness and vertigo. It is not uncommon for patients to experience some improvement shortly after starting the medication, as you did within three hours. However, the degree of improvement can vary significantly among individuals.

Regarding the duration of treatment with Cerenin, it is essential to understand that vestibular disorders can be complex and may require a longer course of treatment to achieve optimal results. While some patients may notice significant improvement within a few weeks, others may need to continue the medication for several months to fully benefit from its effects. In general, a treatment duration of three to six months is not unusual, especially if you are experiencing gradual improvement.

It is also important to consider that while Cerenin can help manage symptoms, it may not necessarily "cure" the underlying condition. If your symptoms have plateaued after a month of treatment, it may indicate that your body has reached a level of stabilization, but it does not rule out the possibility of further improvement with continued use. Regular follow-ups with your healthcare provider are crucial to monitor your progress and make any necessary adjustments to your treatment plan.

In addition to medication, other therapeutic approaches may be beneficial in managing dizziness. Vestibular rehabilitation therapy (VRT) is a specialized form of physical therapy designed to help patients improve balance and reduce dizziness through specific exercises. Engaging in VRT can complement your medication regimen and may enhance your overall recovery.

Moreover, lifestyle modifications can also play a role in managing dizziness. Staying hydrated, avoiding sudden head movements, and reducing stress can help mitigate symptoms. If you notice any specific triggers that exacerbate your dizziness, it may be helpful to keep a diary to discuss with your healthcare provider.

In conclusion, while Cerenin has shown some effectiveness in alleviating your symptoms, it may be beneficial to continue the medication for a longer duration, as advised by your physician. Regular follow-ups and possibly incorporating vestibular rehabilitation therapy could further aid in your recovery. Always communicate openly with your healthcare provider about your symptoms and any concerns you may have regarding your treatment plan.
