Chronic abdominal bloating
Hello Dr.
Chen,
Many years ago, I suffered from gastric and duodenal ulcers.
Since then, I experience gas and abdominal bloating after meals, particularly after consuming meat, legumes, eggs, fermented foods, glutinous rice products, yogurt, and dairy products.
The gas is often foul-smelling, resembling the odor of feces.
This issue has not improved to this day.
In recent years, I have consulted with gastroenterologists at a medical center.
They advised that as long as I maintain a normal lifestyle, exercise regularly, reduce stress, drink plenty of water, and take medications to reduce bloating, my symptoms would improve.
I have followed their advice, but my condition remains unchanged.
I also consulted with a colorectal specialist and underwent a colonoscopy.
During the procedure, the doctor noted that I am thin, and I experienced severe pain when the instrument reached my navel.
However, the examination from the rectum to the navel showed normal results.
The colorectal specialist suggested that I could undergo gastrointestinal imaging to examine the area above the navel.
Therefore, he recommended switching the examination to a sigmoidoscopy instead of a colonoscopy.
I would like to ask you, since neither Western nor traditional Chinese medicine has been able to cure my symptoms, what kind of examination should I undergo to thoroughly address the issues of gas and abdominal bloating? This problem has also affected my interpersonal relationships.
Please help me, thank you!

More Info
Chronic bloating and gastrointestinal issues can be quite distressing, especially when they persist despite various treatments and lifestyle modifications. Based on your description, it seems that your symptoms are multifaceted, involving a history of peptic ulcers, dietary triggers, and ongoing discomfort after meals. Here’s a comprehensive approach to understanding and managing your condition.
Understanding Your Symptoms
1. Dietary Triggers: You mentioned that certain foods, particularly meat, beans, fermented products, and dairy, exacerbate your bloating. This could indicate a food intolerance or sensitivity, particularly to lactose (found in dairy) or certain carbohydrates (like those in beans and some grains). The foul-smelling gas you describe could be due to fermentation of undigested food in the intestines, which can occur when the gut microbiome is imbalanced or when there is insufficient digestive enzyme activity.
2. Previous Conditions: Your history of gastric and duodenal ulcers suggests that your gastrointestinal tract has experienced significant stress. While these ulcers may have healed, the underlying sensitivity of your gut could still be present, leading to dysregulation in gut motility and digestion.
3. Functional Gastrointestinal Disorders: Conditions like Irritable Bowel Syndrome (IBS) could be at play here, especially since you experience bloating and gas. IBS is characterized by a combination of abdominal pain and altered bowel habits, which can include bloating and gas production.
Recommended Diagnostic Approach
Given your persistent symptoms and the previous investigations you've undergone, here are some steps you might consider:
1. Comprehensive Gastrointestinal Evaluation: Since your symptoms persist despite previous evaluations, a more thorough investigation may be warranted. This could include:
- Upper GI Series or Small Bowel Imaging: This can help visualize the upper gastrointestinal tract and identify any structural abnormalities that may not have been seen in previous tests.
- Hydrogen Breath Tests: These tests can help diagnose carbohydrate malabsorption issues, such as lactose intolerance or fructose malabsorption, which can lead to bloating and gas.
- Stool Tests: To check for malabsorption, infections, or imbalances in gut flora.
2. Dietary Assessment: Working with a registered dietitian can be beneficial. They can help you identify specific food triggers through an elimination diet and guide you in reintroducing foods to pinpoint what exacerbates your symptoms.
3. Gut Microbiome Analysis: Some clinics offer tests that analyze the gut microbiome, which can provide insights into dysbiosis (an imbalance of gut bacteria) that may contribute to your symptoms.
Management Strategies
1. Dietary Modifications:
- Low FODMAP Diet: This diet restricts certain fermentable carbohydrates that can cause bloating and gas. It may be worth trying under the guidance of a dietitian.
- Probiotics: These can help restore balance to your gut microbiome. However, the effectiveness can vary, so it may take some trial and error to find the right strain for you.
2. Lifestyle Changes:
- Regular Exercise: Physical activity can help improve gut motility and reduce bloating.
- Stress Management: Since stress can exacerbate gastrointestinal symptoms, incorporating relaxation techniques such as yoga, meditation, or deep breathing exercises may be beneficial.
3. Medications: If dietary and lifestyle changes do not provide relief, discussing medication options with your healthcare provider may be necessary. This could include:
- Antispasmodics: To help relieve abdominal cramping.
- Digestive Enzymes: To assist in breaking down food more effectively.
Conclusion
Chronic bloating can be a complex issue that requires a multifaceted approach. Given your history and the persistence of your symptoms, it is essential to work closely with healthcare professionals who can guide you through further evaluations and tailored management strategies. By identifying specific triggers and implementing dietary and lifestyle changes, you may find significant relief from your symptoms, improving both your physical comfort and quality of life.
