Unexplained symptoms
Hello Doctor: Two years ago, I experienced dizziness three times and underwent various tests at nearby hospitals, all of which were normal.
Since that experience, I often feel very fatigued, with a heavy head and light legs, and I feel dizzy.
I am very afraid of experiencing dizziness again.
Yesterday, after a nocturnal emission, I suddenly felt extremely tired, dizzy, and nauseous.
I was very scared and felt like I was about to pass out.
I thought that if I just slept, I would feel better, but I was completely unable to fall asleep and felt very panicked.
I am unsure whether this is a psychological issue or if there is a physical problem.
Please help me, Doctor...
(It has been over a year, and I have undergone tests at the hospital without any abnormalities, but my body continues to experience discomfort.)

Mr.
Rock: Hello, regarding your discomfort, it indeed aligns with symptoms of anxiety disorder.
Typical manifestations include muscle tension, widespread aches, mood swings, difficulty concentrating, insomnia, and restlessness, among others.
Of course, these symptoms must persist for at least several months to be classified as anxiety disorder.
To understand the course of the condition, it is essential to identify the sources of stress, which may stem from concerns about dizziness, but could also be due to other reasons you haven't mentioned.
If there are identifiable stressors, the condition should improve once those stressors are removed; however, in some cases, a specific stressor cannot be identified, which is referred to as generalized anxiety disorder and may require longer-term treatment.
Anxiety disorders can present in various ways, with panic disorder being particularly distressing, where one may suddenly feel like they are about to collapse within a few minutes.
This condition requires treatment, as it can significantly impact daily life due to fear of episodes.
Short-term treatment can lead to noticeable improvement with the use of anxiolytic medications, but for long-term medication management, it is best to combine it with behavioral therapy.
Therefore, it is advisable to seek evaluation and arrangement from a psychiatrist, who specializes in mental health.
As for dizziness, there are many potential causes, with viral infections being more common in younger individuals, while less common causes include Meniere's disease, among others.
It is best to consult an otolaryngologist to rule out these possibilities, as excessive worry about dizziness can lead to additional stress.
I hope this response is helpful to you! Wishing you peace and health.
Sincerely, Dr.
Tsai Meng-Ju, Attending Physician, Family Medicine Department, Taoyuan General Hospital, Ministry of Health and Welfare.

Understanding unexplained symptoms can be a daunting experience for many patients, especially when traditional medical evaluations yield no clear diagnosis. Your situation, characterized by recurrent dizziness, fatigue, and a sense of impending doom, is understandably distressing. It’s crucial to approach this matter with both medical insight and psychological support.
First, it’s important to recognize that unexplained symptoms can arise from a variety of sources, including physical, psychological, or a combination of both. In your case, the dizziness and fatigue you've experienced after multiple medical evaluations suggest that there may be underlying factors that have not yet been identified. Conditions such as anxiety disorders, depression, or even somatic symptom disorder can manifest as physical symptoms without any identifiable medical cause. This is often referred to as a "conversion disorder," where psychological distress is expressed through physical symptoms.
Given that you have undergone extensive testing without any significant findings, it may be beneficial to consider the role of stress and anxiety in your symptoms. Anxiety can lead to physical manifestations such as dizziness, fatigue, and a feeling of being unwell. The fear of experiencing dizziness again can create a cycle of anxiety that exacerbates your symptoms. Cognitive-behavioral therapy (CBT) is one effective approach that can help you manage anxiety and break this cycle. It focuses on changing negative thought patterns and behaviors that contribute to your distress.
Additionally, it’s essential to explore lifestyle factors that may be contributing to your symptoms. Poor sleep quality, inadequate nutrition, and lack of physical activity can all lead to feelings of fatigue and dizziness. Establishing a regular sleep schedule, engaging in light physical activity, and maintaining a balanced diet can help improve your overall well-being. Mindfulness practices, such as meditation or yoga, can also be beneficial in reducing anxiety and promoting relaxation.
If you find that your symptoms persist despite these interventions, it may be worthwhile to consult with a mental health professional who specializes in psychosomatic disorders. They can provide a comprehensive evaluation and develop a tailored treatment plan that addresses both your physical and psychological needs.
In terms of medication, if you are currently taking any, it’s important to discuss with your healthcare provider whether they could be contributing to your feelings of fatigue or dizziness. Some medications can have side effects that mimic or exacerbate these symptoms. Your doctor may consider adjusting your medication or exploring alternative options.
Lastly, it’s crucial to maintain open communication with your healthcare providers. If you feel that your concerns are not being adequately addressed, don’t hesitate to seek a second opinion or ask for a referral to a specialist. Your health is paramount, and you deserve a comprehensive approach to your care.
In summary, while unexplained symptoms can be frustrating and frightening, understanding the potential psychological components, making lifestyle adjustments, and seeking appropriate mental health support can significantly improve your situation. Remember, you are not alone in this journey, and there are resources available to help you navigate these challenges.
