What happened?
Hello, doctor: I would like to ask you something.
I have previously experienced symptoms of palpitations and chest tightness, and even chest pain during exercise.
Last Saturday, when I went hiking with relatives, after just a short distance, I felt like my heart was about to jump out of my chest.
I could clearly feel its rhythm; it was beating twice and then pausing for three beats, continuing this pattern throughout the hike.
This caused me severe chest pain, difficulty breathing, and dizziness.
Is this considered normal? Should I seek medical attention?

1.
Palpitation refers to the sensation of an irregular heartbeat, rapid heart rate, occasional heart stoppage, or an irregular heartbeat experienced by the patient.
Common causes include heart diseases, mental health disorders, physiological responses, medication reactions, or other conditions such as arrhythmias, anxiety disorders, thyroid diseases, and drug-induced effects.
2.
Generally, if palpitations are accompanied by dizziness, fainting, near-fainting, shortness of breath, or chest pain, they should be treated as a medical emergency.
3.
It is advisable to first discuss your symptoms with your primary care physician or seek treatment directly from a cardiology clinic.

More Info
Hello! Thank you for reaching out with your concerns regarding your heart palpitations and associated symptoms. Based on the information you've provided, it sounds like you are experiencing significant cardiovascular symptoms that warrant further evaluation.
Heart palpitations can be described as the sensation of feeling your heart beating, which can sometimes feel like it is racing, pounding, or fluttering. While palpitations can be benign and often occur in healthy individuals, the symptoms you are experiencing—especially in conjunction with chest pain, shortness of breath, and dizziness—are concerning and should not be taken lightly.
Given your history of chest tightness and the recent episode during physical activity, it is crucial to consider the possibility of underlying heart conditions. The fact that you felt your heart "jumping" and experienced a pattern of "two beats, then a pause" could indicate an arrhythmia, which is an abnormal heart rhythm. This can sometimes be harmless, but it can also be a sign of more serious conditions, such as atrial fibrillation or other types of arrhythmias that may require medical intervention.
The symptoms of chest pain and difficulty breathing, especially when they occur during exertion, are particularly concerning for potential ischemic heart disease, which is when the heart muscle does not receive enough blood and oxygen. This could lead to angina (chest pain due to reduced blood flow) or even a heart attack in severe cases. Your description of feeling faint or dizzy further emphasizes the need for immediate medical attention, as these symptoms can indicate that your heart is not pumping effectively.
In light of your symptoms, I strongly recommend that you seek medical attention as soon as possible. A healthcare provider will likely perform a thorough evaluation, which may include:
1. Electrocardiogram (ECG): This test records the electrical activity of your heart and can help identify arrhythmias or other heart conditions.
2. Echocardiogram: An ultrasound of the heart can assess its structure and function, including how well the heart is pumping blood.
3. Stress Testing: This may be done to evaluate how your heart performs under physical stress, which can help identify any ischemic changes.
4. Holter Monitor: If your palpitations are intermittent, a Holter monitor may be used to record your heart's activity over 24-48 hours to capture any abnormal rhythms.
5. Blood Tests: These can check for markers of heart damage, electrolyte imbalances, or other conditions that might contribute to your symptoms.
In summary, your symptoms of palpitations, chest pain, shortness of breath, and dizziness during physical activity are concerning and should not be ignored. It is essential to consult with a healthcare professional who can provide a comprehensive evaluation and determine the appropriate course of action. Early diagnosis and treatment can significantly improve outcomes, especially if there is an underlying heart condition. Please prioritize your health and seek medical attention promptly.
