Hair loss concerns?
Dear Doctor,
I have been experiencing significant hair loss for a long time, with hair loss occurring all over my scalp.
Every time I wash my hair, I lose between 5 to 60 strands, sometimes even more, which is quite distressing.
When I wake up, I find about 5 to 15 strands on my pillow, and the floor is no exception.
I have also noticed that my scalp is becoming visibly more exposed, and my hair has become very fragile, falling out easily.
Sometimes even hair that has just grown or has been growing for 2 to 3 months falls out easily.
The new hair that grows is very thin, and according to online information, the number of hairs lost should eventually grow back.
However, so far, nothing has regrown, and if it does, it is only fine hair that does not grow long.
If I go two days without washing my hair, I develop a lot of scalp flakes, and even after washing, my scalp still itches.
Sometimes, I also get pimples on my scalp, which can become red, swollen, and bleed.
Occasionally, I feel a tightness in my scalp.
I have an oily scalp.
What could be the issue? I read online that a bacterial infection might be causing the hair loss and preventing hair growth.
Is that true? What should I do? Please help me, Doctor! Do you have office hours on weekends? Should I go for an examination? I'm really distressed!

Dr. Jiang Yujiao reply Dermatology
Dear Jungle: Hello, in terms of the hair growth cycle, it is normal to lose about one hundred hairs a day, which means losing one hundred hairs daily is typical.
However, you may be suffering from seborrheic dermatitis, which can increase hair loss.
Given your age, you have not yet reached the typical age for male pattern baldness.
You may consider using an anti-dandruff shampoo.
Ensuring adequate sleep, reducing the intake of greasy foods, and exercising more can help promote hair regrowth.
Common bacterial folliculitis does not cause scarring, and the hair follicles are not destroyed.
On Saturday (September 5th) morning, I have a clinic appointment.
Wishing you peace and health.
Sincerely, Dr.
Jiang Yujiao, Director of Dermatology, Taoyuan Hospital, Health Department.

More Info
Hair loss can be a distressing experience, especially when it becomes noticeable and affects your self-esteem. Based on your description, it sounds like you are experiencing significant hair loss, which could be attributed to a variety of factors. Let's break down the potential causes, symptoms, and treatment options available to you.
Causes of Hair Loss
1. Genetic Factors: One of the most common causes of hair loss is androgenetic alopecia, also known as male or female pattern baldness. This hereditary condition can lead to thinning hair and bald spots.
2. Hormonal Changes: Hormonal imbalances, such as those caused by pregnancy, menopause, or thyroid issues, can also contribute to hair loss.
3. Stress and Lifestyle: High levels of stress, irregular sleep patterns, and poor nutrition can negatively impact hair health. Stress can trigger a condition called telogen effluvium, where hair follicles enter a resting phase and shed more than usual.
4. Scalp Conditions: Conditions like seborrheic dermatitis, psoriasis, or fungal infections can lead to inflammation and hair loss. The symptoms you described, such as itching, redness, and the presence of scalp acne, may indicate an underlying scalp condition that requires attention.
5. Chemical Damage: Frequent dyeing or using harsh hair products can damage hair and lead to breakage and loss.
Symptoms to Monitor
- Excessive Shedding: Losing 50-100 hairs a day is normal, but your experience of losing 5-60 hairs during washing and finding hair on your pillow is concerning.
- Thinning Hair: The visibility of your scalp and the texture of your hair becoming finer are signs of hair loss.
- Scalp Issues: Itchy, red, or inflamed scalp can indicate a dermatological issue that needs to be addressed.
Treatment Options
1. Consult a Dermatologist: Given the severity of your symptoms, it is crucial to see a dermatologist. They can perform a thorough examination, possibly including blood tests or a scalp biopsy, to determine the underlying cause of your hair loss.
2. Medications: Depending on the diagnosis, treatments may include:
- Minoxidil (Rogaine): An over-the-counter topical treatment that can stimulate hair growth and slow down hair loss.
- Finasteride (Propecia): A prescription medication for men that can help prevent further hair loss.
- Corticosteroids: If inflammation is present, corticosteroids may be prescribed to reduce scalp inflammation.
3. Lifestyle Changes: Improving your diet by incorporating more vitamins and minerals, particularly those known to support hair health (like biotin, zinc, and iron), can be beneficial. Additionally, managing stress through relaxation techniques, regular exercise, and ensuring adequate sleep can help.
4. Scalp Care: Use gentle, sulfate-free shampoos and avoid excessive heat styling or harsh chemical treatments. Regularly exfoliating the scalp can help remove buildup and promote a healthier environment for hair growth.
5. Hair Restoration Treatments: In more severe cases, options such as PRP (platelet-rich plasma) therapy or hair transplant surgery may be considered.
Conclusion
It’s understandable to feel anxious about your hair loss, especially when it seems to be worsening. The symptoms you describe warrant a professional evaluation to rule out any serious conditions and to develop an appropriate treatment plan. Make sure to schedule an appointment with a dermatologist as soon as possible. They can provide you with tailored advice and treatment options based on your specific situation. Remember, early intervention is key in managing hair loss effectively.
