Hair loss (Bokangsi) issues?

Hello Dr.
Tsai.
Is it effective for overall hair growth to take Biotin supplements? I currently do not have a Mediterranean condition, but I have significant hair loss in the frontal area.
Is there hope for regrowth with long-term use of Biotin? Thanks, Jason.

Jason, 30~39 year old female. Ask Date: 2008/09/17

Hello: If I remember correctly, Propecia is the brand name for finasteride in Hong Kong.
To determine if it is effective for you, it is essential to rule out other types of hair loss disorders and confirm that your hair loss is due to androgenetic alopecia.
Generally, it takes at least three to six months of treatment with Propecia to see results, and once you start using it, it is not recommended to discontinue the medication, as you may revert to your original hair loss pattern within six months.
This is unless you no longer care about the hair loss.
It is also important to note that Propecia tends to be more effective for androgenetic alopecia than for receding hairlines.
Additionally, you may consider using Minoxidil topical solution in conjunction with oral finasteride.
I recommend that you seek medical advice to confirm your diagnosis before starting any medication for safety.
Best wishes, Dr.
Tsai Chang-Lin, Department of Dermatology and Cosmetic Center, Keelung Hospital, Ministry of Health and Welfare.

More Info

Finasteride, commonly known by its brand name Propecia, is a medication that has been widely used for the treatment of male pattern baldness (androgenetic alopecia). It works by inhibiting the enzyme 5-alpha-reductase, which converts testosterone into dihydrotestosterone (DHT). DHT is a hormone that is primarily responsible for hair loss in genetically predisposed individuals. By reducing the levels of DHT in the scalp, finasteride can help to slow down hair loss and, in some cases, promote hair regrowth.

In your case, since you are experiencing significant hair loss at the frontal area, finasteride could potentially be beneficial. Clinical studies have shown that finasteride can lead to an increase in hair count and improvement in hair appearance for many men experiencing androgenetic alopecia. However, the effectiveness of finasteride can vary from person to person. Some individuals may see noticeable results within a few months, while others may take longer or may not respond as well.

It's important to note that finasteride is generally well-tolerated, but it can have side effects. The most commonly reported side effects include decreased libido, erectile dysfunction, and, in some cases, breast tenderness or enlargement. These side effects are relatively rare, but they can be concerning for some users. If you experience any persistent side effects, it is crucial to discuss them with your healthcare provider.

Regarding your question about the long-term use of finasteride and its effectiveness, many users find that continued use is necessary to maintain the benefits. If you stop taking the medication, any hair regrowth achieved may be lost, and hair loss may resume. Therefore, consistency is key when using finasteride for hair loss treatment.

In addition to finasteride, you mentioned the use of other supplements such as biotin and pumpkin seed oil. Biotin is a B vitamin that is often marketed for hair health, but scientific evidence supporting its effectiveness for hair growth in individuals without a deficiency is limited. Pumpkin seed oil has been suggested to have some benefits for hair growth, possibly due to its phytosterol content, which may help block DHT. However, more research is needed to establish its efficacy definitively.

If you are considering combining finasteride with other treatments, such as minoxidil (a topical treatment), this can be an effective strategy. Minoxidil works through a different mechanism and can complement the effects of finasteride. Many individuals use both treatments together to maximize their chances of hair regrowth.

In summary, finasteride can be an effective treatment for male pattern baldness, particularly for those experiencing hair loss at the frontal area. Long-term use is often necessary to maintain results, and while side effects can occur, they are not experienced by everyone. If you have concerns about your hair loss or the medications you are taking, it is advisable to consult with a healthcare professional or a dermatologist who specializes in hair loss. They can provide personalized recommendations based on your specific situation and help you navigate the various treatment options available.

Dr. Wu, I take 1/4 of Finasteride daily, along with 10,000 micrograms of Biotin, 1,000 milligrams of Pumpkin Seed Oil, and Minoxidil. Will this help with hair growth, or is it excessive? Since they all have DHT-blocking effects, could there be any side effects?

It can be helpful; the side effects are mainly related to oral administration, and a small percentage of individuals may experience decreased sexual function as a side effect.
