Symptoms: Could They Indicate Heart Failure? - Cardiology

Suspected heart failure?

Hello Doctor: I am currently 43 years old, with a smoking history of about 15 years.
I occasionally drink a little alcohol, but I quit smoking and drinking three years ago.
My medical history includes arrhythmia (I have undergone a 24-hour Holter monitor) accompanied by palpitations, and my heart has stopped beating for about 15 seconds before resuming.
My thyroid function level is at the upper limit of 1.8.
About a year ago, I had an echocardiogram and X-ray, and the doctor said everything was normal and that my heart was not enlarged.

Question 1: In the past two months, I have experienced multiple episodes of waking up suddenly during sleep (after having fallen asleep), seemingly due to a cessation of breathing, followed by rapid breathing (shortness of breath), which lasts for about ten seconds before returning to normal.

Question 2: When I run (approximately 600-800 meters of jogging), I feel particularly short of breath, my face becomes very flushed, and my heart rate increases.

I would like to ask the doctor if my symptoms could potentially be a precursor to heart failure, or if they might indicate another condition.
How should I seek medical attention? I sincerely request your guidance, thank you! Wishing you all the best and good health.

Hello: 1.
The description of the first issue is similar to sleep apnea syndrome.
Some hospitals have sleep study centers in their neurology departments, where you can seek consultation.
2.
The description of the second issue does not align very well with the typical clinical symptoms of heart failure.
It is recommended to follow up at a cardiology outpatient clinic.
Sincerely, Dr.
Lin Chi-Hsiung, Taoyuan General Hospital.

Understanding your symptoms and concerns is crucial, especially when it comes to potential heart issues such as heart failure. Based on the information you've provided, let's break down your symptoms and explore their possible implications.

Firstly, heart failure is a condition where the heart is unable to pump blood effectively, leading to insufficient blood flow to meet the body's needs. This can result in symptoms such as shortness of breath, fatigue, and fluid retention. However, it's important to note that not all symptoms of shortness of breath or fatigue indicate heart failure. Other conditions, including respiratory issues, anxiety, or even sleep apnea, can also present similar symptoms.

Your history of arrhythmia and the episodes of your heart stopping for about 15 seconds are significant. These could indicate underlying heart issues that may predispose you to heart failure or other cardiovascular problems. The fact that you have experienced episodes of waking up gasping for air could suggest sleep apnea, a condition where breathing repeatedly stops and starts during sleep, which can lead to increased strain on the heart over time.

Regarding your second concern about experiencing shortness of breath and facial flushing during exercise, this could be related to several factors. While it may not directly indicate heart failure, it could suggest that your heart is struggling to meet the increased demands during physical activity. This could be due to a variety of reasons, including deconditioning, underlying heart issues, or even anxiety.

Given your symptoms and medical history, it is advisable to consult a cardiologist for a thorough evaluation. They may recommend further tests, such as an echocardiogram, stress test, or even a sleep study, to assess your heart function and rule out conditions like sleep apnea. It's essential to provide your healthcare provider with a complete picture of your symptoms, including the frequency and duration of your episodes, to aid in accurate diagnosis.

In summary, while your symptoms could potentially indicate heart failure, they may also be related to other conditions. A comprehensive evaluation by a cardiologist is crucial to determine the underlying cause of your symptoms and to develop an appropriate treatment plan. Early intervention can significantly improve outcomes, especially if there are underlying cardiovascular issues. Remember, it’s always better to err on the side of caution when it comes to heart health.
