Penile curvature issues and penile fracture problems?

I apologize, Director Hsu, and thank you for your patience.
I just wrote a message but realized I forgot to ask many things.
I have a downward curvature of the penis.
I discovered in the past five years that I have difficulty achieving an erection, as well as issues with premature ejaculation.
I didn't have these problems before.
However, I noticed that when my penis is slightly erect, if I pull it upwards to straighten it, it becomes very firm.
Should I consider surgery to correct this? Would it resolve these issues?
Additionally, about 10 to 7 years ago, I experienced a minor sound during an activity that felt like a fracture.
However, I did not go to the hospital at that time.
Could this have caused erectile dysfunction? If so, how can I check for a penile fracture? Is it still possible to assess it after such a long time? I am quite concerned, which is why I have so many questions.
Thank you.

Hello, penile fracture can cause significant bleeding and result in a deformation of the penis, making it appear like a "saxophone." This usually occurs due to severe external force (such as in the woman-on-top position).
It is difficult to cause a penile fracture simply by manual manipulation.
Occasionally, there may be a sound, which could just be the sound of the penile fascia rubbing; this is generally not a cause for concern.
If there are any unclear points, feel free to ask again for further discussion and research.
Penile curvature does not always require surgery; typically, if the curvature is less than 30 degrees and does not affect sexual intercourse, surgery is not necessary.
If there are difficulties achieving an erection, it may be worth considering whether there is insufficient stimulation or if one has become accustomed to that level of stimulation.
Additionally, if there are frequent issues with erections, it may involve endocrine problems that require further testing.

Understanding penile curvature and potential fracture issues can be complex, and it's important to approach this topic with sensitivity and care. Based on your description, it seems you are experiencing a combination of penile curvature, erectile difficulties, and concerns about past injuries that may have affected your sexual health.

Penile Curvature
Penile curvature can occur for various reasons, including congenital conditions (like congenital penile curvature), Peyronie's disease (which involves the development of fibrous scar tissue inside the penis), or as a result of trauma. If your penis curves downward, it may be affecting your ability to achieve or maintain an erection, as you mentioned.
In some cases, surgical intervention may be considered to correct significant curvature, especially if it interferes with sexual function or causes pain. Surgical options can include procedures to remove the plaque associated with Peyronie's disease or techniques to shorten the longer side of the penis to straighten it. However, surgery is typically recommended only after a thorough evaluation by a urologist, who can assess the severity of the curvature and its impact on your sexual health.

Erectile Dysfunction and Premature Ejaculation
The issues you are experiencing with erectile dysfunction (ED) and premature ejaculation (PE) can be multifactorial. Psychological factors, hormonal imbalances, and physical conditions can all contribute to these problems. The fact that you notice an improvement in rigidity when you manually straighten your penis suggests that the curvature may be a contributing factor to your ED.
It's essential to consult a healthcare professional who specializes in sexual medicine or urology. They can perform a detailed assessment, which may include blood tests to check hormone levels, a physical examination, and possibly imaging studies to evaluate the structure of the penis.

Past Injuries and Potential Fracture
Regarding your past injuries, it’s important to clarify that the penis does not have bones in the traditional sense; instead, it contains erectile tissue that can be injured. The term "penile fracture" refers to a rupture of the tunica albuginea, the fibrous covering of the erectile tissue, which can occur during vigorous sexual activity or trauma.
If you heard a "popping" sound during sexual activity, it could indicate a penile fracture, especially if you experienced immediate pain, swelling, or bruising. However, if you did not seek medical attention at the time and have not experienced ongoing issues since, it is less likely that this injury is the cause of your current erectile problems.
To evaluate any potential long-term effects of past trauma, a urologist may perform a physical examination and possibly imaging studies, such as an ultrasound, to assess the integrity of the erectile tissue and blood flow.

Recommendations
1. Consult a Urologist: Given your symptoms and concerns, it is crucial to see a urologist who can provide a comprehensive evaluation. They can discuss your penile curvature, erectile dysfunction, and any past injuries in detail.

2. Consider Psychological Factors: Sometimes, psychological factors can contribute to ED and PE. Therapy or counseling may be beneficial, especially if anxiety or stress is involved.

3. Explore Treatment Options: Depending on the findings, treatment options may include medications for ED, penile rehabilitation exercises, or surgical options for curvature.

4. Follow-Up on Past Injuries: If you are concerned about the effects of past injuries, bring this up during your consultation. The urologist can determine if any further evaluation or treatment is necessary.

In conclusion, while penile curvature and erectile dysfunction can be distressing, there are various treatment options available. A thorough evaluation by a healthcare professional will help determine the best course of action tailored to your specific situation. Don't hesitate to reach out for help; addressing these concerns can significantly improve your quality of life.
