About the menstrual cycle?

My menstrual cycle is approximately 25 to 26 days, but sometimes it occurs every 23 to 24 days, and this month it was only 22 days since the last one.
Is this normal? What could be causing the shortening of the cycle?

GL, 30~39 year old female. Ask Date: 2009/01/12

Dr. Huang Jianxun reply Obstetrics and Gynecology

Hello: A menstrual cycle shorter than 21 days is considered abnormal, with the most common cause being hormonal imbalance.
It is advisable to seek medical attention at a hospital.
Wishing you peace and health.
Sincerely, Dr.
Huang Chien-Hsun, Director of the Department of Obstetrics and Gynecology, Taoyuan Hospital, Ministry of Health and Welfare.

Irregular menstrual cycles can be a source of concern for many women, and understanding the underlying causes is essential for addressing any potential health issues. A typical menstrual cycle ranges from 21 to 35 days, with an average of about 28 days. However, variations are common, and cycles can fluctuate due to various factors.

In your case, experiencing cycles that range from 22 to 26 days is not necessarily abnormal, but it does indicate a shift in your menstrual pattern. Several factors can contribute to changes in menstrual cycle length, including hormonal imbalances, stress, lifestyle changes, and underlying medical conditions.

1. Hormonal Imbalances: The menstrual cycle is regulated by a complex interplay of hormones, primarily estrogen and progesterone. Any disruption in the balance of these hormones can lead to irregularities. Conditions such as polycystic ovary syndrome (PCOS), thyroid disorders, and perimenopause can cause hormonal fluctuations that affect cycle length.

2. Stress: Psychological stress can significantly impact your menstrual cycle. Stress triggers the release of cortisol, which can interfere with the hormones that regulate your cycle. If you have been experiencing increased stress levels, this could be a contributing factor to your shorter cycles.

3. Lifestyle Factors: Changes in weight, diet, and exercise can also influence your menstrual cycle. Significant weight loss or gain, intense physical activity, or changes in your eating habits can lead to irregular cycles. For instance, athletes or those who engage in rigorous exercise may experience changes in their menstrual patterns due to low body fat or energy availability.

4. Medical Conditions: Certain medical conditions can lead to irregular menstrual cycles. Conditions such as endometriosis, uterine fibroids, and pelvic inflammatory disease can cause changes in menstrual patterns. If you have any accompanying symptoms, such as severe pain, heavy bleeding, or unusual discharge, it is essential to consult a healthcare provider.

5. Age: As women approach their late 30s and 40s, they may begin to experience changes in their menstrual cycles as they transition into perimenopause. This phase can be characterized by irregular cycles, changes in flow, and other symptoms.

If you are concerned about your menstrual cycle length, it is advisable to keep a detailed record of your cycles, noting any accompanying symptoms, lifestyle changes, or stressors. This information can be helpful for your healthcare provider in diagnosing any potential issues.

In conclusion, while a menstrual cycle that shortens to 22 days can be concerning, it is not uncommon and can be influenced by various factors. If the changes persist or if you experience other symptoms, it is crucial to seek medical advice. A healthcare provider can perform a thorough evaluation, including hormonal assessments and possibly imaging studies, to determine the underlying cause and recommend appropriate treatment or management strategies. Regular check-ups and open communication with your healthcare provider can help ensure your reproductive health remains optimal.
