Dizziness caused by a car accident?
Hello, Doctor Gao: About a week ago, I was involved in a car accident where I hit my head and also compressed my neck.
Initially, I experienced headaches and dizziness.
After visiting the hospital, I was informed that I might have a mild concussion.
I took pain relievers and anti-nausea medication a few times, which alleviated the pain; however, I have noticed that when I lie down, turn over in bed, get up, or sit down, any slight movement of my head causes intense dizziness.
I am unsure if this is normal and whether it could have adverse effects on my brain function.
Will this condition gradually improve? What should I be cautious about? Thank you!

Dr. Gao Chunde reply Neurology
Hello, the preliminary assessment suggests that the issue is likely due to disturbances in the vestibular system and semicircular canals of the inner ear caused by the deceleration from a head impact.
Some also mention the possibility of otolith dislodgement or displacement.
Generally, this condition tends to resolve gradually on its own, but it can also be corrected through repositioning maneuvers performed by a specialized otolaryngologist.
Therefore, theoretically, it is unrelated to cognitive functions of the brain.

More Info
Post-car accident dizziness can be a concerning symptom, especially when it follows a head injury. In your case, it seems that you experienced a mild concussion, which is a common result of head trauma. The symptoms you describe—headaches, dizziness, and sensations of spinning (vertigo)—are typical manifestations of a concussion or post-concussion syndrome.
Understanding Symptoms
1. Dizziness and Vertigo: After a head injury, the brain can be sensitive to movement, which may cause dizziness or a spinning sensation when you change positions, such as lying down, getting up, or turning over in bed. This is often due to disturbances in the vestibular system, which helps control balance.
2. Headaches: These can be a direct result of the concussion or may stem from muscle tension in the neck and shoulders, especially if you also experienced neck compression during the accident.
3. Post-Concussion Syndrome: This condition can occur after a concussion and may include prolonged symptoms such as dizziness, headaches, fatigue, and cognitive difficulties. It can last for weeks or even months after the initial injury.
Recovery and Management
1. Rest: It is crucial to allow your brain to heal. This means getting plenty of rest and avoiding activities that could exacerbate your symptoms, such as strenuous exercise, screen time, or anything that requires intense concentration.
2. Gradual Return to Activities: As your symptoms improve, you can gradually reintroduce activities. Start with light activities and slowly increase intensity as tolerated. Pay attention to how your body responds.
3. Hydration and Nutrition: Staying well-hydrated and maintaining a balanced diet can support your recovery. Dehydration can worsen headaches and dizziness.
4. Physical Therapy: If dizziness persists, consider consulting a physical therapist who specializes in vestibular rehabilitation. They can provide exercises to help improve balance and reduce dizziness.
5. Follow-Up Care: It’s essential to have follow-up appointments with your healthcare provider. They can monitor your symptoms and adjust your treatment plan as necessary. If symptoms worsen or new symptoms develop, seek medical attention promptly.
When to Seek Immediate Help
- If you experience severe headaches that do not respond to medication.
- If dizziness worsens to the point where you cannot perform daily activities.
- If you have difficulty walking or maintaining balance.
- If you experience any new neurological symptoms, such as weakness, numbness, or changes in vision.
Conclusion
In summary, while dizziness and other symptoms following a car accident can be alarming, they are often part of the recovery process from a concussion. With appropriate rest, gradual reintroduction of activities, and possibly physical therapy, many individuals see improvement over time. However, it is essential to stay vigilant and communicate with your healthcare provider about your symptoms and recovery progress. They can provide tailored advice and interventions to ensure a safe and effective recovery.
