Fertility?
Dear Dr.
Hsu,
Hello! I would like to ask you about phimosis and whether it can affect fertility.
I was previously unaware of any abnormalities because I never compared myself with others.
It wasn't until I became a medical student that I gradually learned what phimosis looks like.
It seems that I have phimosis because the glans does not expose during an erection (the glans is still covered by the foreskin), but there is no pain! I would like to ask you, as a professional, if phimosis can affect fertility? Is it possible to avoid surgery? Can I manage it by regularly retracting and cleaning? (I just had hernia repair surgery in early March...) I would love to hear your thoughts.
Thank you very much!

Fearful medical students; 1.
Phimosis does not affect fertility, as long as semen can flow out, conception can occur successfully.
2.
As long as one maintains regular cleaning and retraction, the risk of infection will not increase, and surgery may not be necessary.
If there are any unclear points, feel free to ask again for further discussion and research.

Phimosis, a condition where the foreskin cannot be easily retracted over the glans (head) of the penis, can raise concerns regarding fertility, although its direct impact is often debated among medical professionals. Understanding the implications of phimosis on fertility requires a closer look at both the physiological aspects and the potential complications associated with this condition.
Firstly, it is important to note that phimosis itself does not inherently prevent a man from fathering children. Many men with phimosis can still achieve ejaculation and have viable sperm. However, there are several indirect factors related to phimosis that could potentially affect fertility:
1. Hygiene Issues: One of the primary concerns with phimosis is the difficulty in maintaining proper hygiene. If the foreskin cannot be retracted, it can lead to the accumulation of smegma (a combination of dead skin cells and oils) under the foreskin. This can create an environment conducive to infections, such as balanitis (inflammation of the glans) or other sexually transmitted infections (STIs). Infections can negatively impact sperm quality and overall reproductive health.
2. Sexual Function: While many men with phimosis do not experience pain during erections, some may have discomfort or difficulty during sexual intercourse. This can lead to reduced sexual activity, which indirectly affects fertility due to fewer opportunities for conception.
3. Potential for Complications: In severe cases of phimosis, where the foreskin is very tight, it may lead to paraphimosis (where the retracted foreskin cannot return to its normal position) or other complications that might require surgical intervention. If surgery is necessary, it could temporarily affect sexual function and fertility.
4. Psychological Factors: The awareness of having phimosis can lead to anxiety or embarrassment, which may affect sexual performance and desire. Psychological factors can play a significant role in fertility, as stress and anxiety can impact libido and overall sexual health.
Regarding your question about whether surgery is necessary, it depends on the severity of the phimosis and the symptoms experienced. If you are able to maintain good hygiene and do not experience pain or discomfort, some healthcare providers may suggest a conservative approach, such as regular cleaning and gentle stretching exercises. However, if phimosis leads to recurrent infections or significant discomfort, circumcision or other surgical options may be recommended.
In conclusion, while phimosis does not directly cause infertility, it can lead to complications that may affect reproductive health. If you have concerns about your condition, it is advisable to consult with a urologist who can provide a thorough evaluation and discuss the best course of action tailored to your specific situation. Regular check-ups and open communication with your healthcare provider can help manage any potential issues related to phimosis and maintain reproductive health.

Hello, doctor. I would like to follow up on my question: if I ignore this and do not seek medical attention, what impact could it have? For example, in terms of future fertility. Thank you.
Dr. Xu Weikai reply Urology
Of course, we can discuss it later. If treatment is not accepted, a tighter foreskin may make cleaning difficult, which could lead to frequent inflammation and subsequently cause foreskin fibrosis (which means it becomes harder). However, if you can take good care of it and there...[Read More] Impact of Ignoring Painful Erection Issues on Future Fertility
