Consistently passing soft stools?
Hello, doctor.
I have been experiencing soft stools for over a month now.
I have seen a doctor and taken medication, but the condition remains the same.
I do not have any abdominal pain, my diet is normal, and I have bowel movements 1 to 2 times a day with a normal amount.
The bowel movements are smooth, and I do not have any difficulty in completely emptying my bowels, nor do I have the urge to defecate without being able to.
However, my stools are consistently soft, sometimes unformed, and occasionally contain undigested food.
I previously took iron supplements and spirulina, and even when I stopped, my stool color remained green.
Ten years ago, I underwent gastric sleeve surgery.
My mother had colorectal cancer, and I have gastroesophageal reflux disease (GERD) for which I have been taking omeprazole long-term.
Could my gastric sleeve surgery have weakened my digestive function, or could it be due to the medication? Is there a possibility that there is an issue with my colon? Do I need to undergo a colonoscopy? Could this be a precursor to colorectal cancer? I would appreciate your answers and suggestions.
Thank you!

Dr. Chen Shidian reply Gastroenterology and Hepatology
Dear Ms.
Donna:
Surgery can affect gastric motility and digestion, and Omelon may also have some impact on the balance of intestinal flora, which can lead to softer stools.
You may want to discuss with your physician about adjusting your medication and dietary habits.

More Info
Persistent soft stools can be a concerning symptom, especially when they last for an extended period, as you have described. Given your medical history, including a previous gastric bypass surgery, gastroesophageal reflux disease (GERD), and a family history of colorectal cancer, it is essential to approach this situation with care and thorough evaluation.
Possible Causes of Persistent Soft Stools
1. Dietary Factors: Your diet plays a significant role in stool consistency. High fiber intake, particularly from fruits, vegetables, and whole grains, can lead to softer stools. Additionally, certain foods, such as dairy products, fatty foods, or artificial sweeteners, can also affect stool consistency. If you've recently changed your diet or have been consuming more of these foods, it could explain the change.
2. Medications: You mentioned taking iron supplements and green algae. Iron supplements can sometimes cause gastrointestinal side effects, including changes in stool color and consistency. Green algae or other supplements may also affect digestion. It's crucial to consider all medications and supplements you are taking, as they can interact and influence your gastrointestinal health.
3. Gastrointestinal Conditions: Given your history of gastric bypass surgery, your digestive system may be more sensitive or altered, leading to malabsorption issues. This could result in undigested food particles in your stool. Conditions such as irritable bowel syndrome (IBS), inflammatory bowel disease (IBD), or infections could also lead to persistent soft stools.
4. Malabsorption Syndromes: After gastric bypass surgery, the body may not absorb nutrients effectively, leading to symptoms like soft stools and undigested food in the stool. This could be due to a lack of digestive enzymes or changes in gut flora.
5. Colon Health: While it is less common, persistent changes in bowel habits can sometimes indicate underlying issues in the colon, including polyps or, in rare cases, colorectal cancer. Given your family history, it is prudent to be vigilant.
When to Seek Help
You should definitely seek medical advice if:
- Your symptoms persist beyond a few weeks without improvement.
- You notice any additional symptoms such as weight loss, blood in your stool, or significant changes in your bowel habits.
- You experience any abdominal pain, cramping, or discomfort.
- You have a family history of colorectal cancer, which warrants a more proactive approach to screening.
Recommended Actions
1. Consult a Gastroenterologist: Given your history and current symptoms, a gastroenterologist can provide a thorough evaluation. They may recommend tests such as a colonoscopy to rule out any serious conditions, especially considering your family history.
2. Dietary Review: Keep a food diary to track what you eat and any changes in your stool. This can help identify any dietary triggers that may be contributing to your symptoms.
3. Medication Review: Discuss all medications and supplements with your healthcare provider to determine if any could be contributing to your symptoms.
4. Consider Probiotics: If malabsorption is suspected, probiotics may help restore healthy gut flora and improve digestion.
5. Regular Monitoring: Keep track of your symptoms and any changes. Regular follow-ups with your healthcare provider can help manage your condition effectively.
In conclusion, while persistent soft stools can be benign, given your medical history and family background, it is essential to take these symptoms seriously. A thorough evaluation by a healthcare professional is warranted to rule out any underlying conditions and to provide appropriate management.
