Itchy?
Hello, I am a 17-year-old student and I have a question.
When I pull down my foreskin, I feel an itch near the ligament below the glans.
This has been going on for a month.
A month ago, I also had itching in the urethra, but that has improved (sometimes I feel a slight pain when urinating).
Now, the itching is primarily around the area near the ligament, and it feels like the skin on the foreskin has become more wrinkled.
What could this be, and what should I do about it? (I have engaged in sexual activity and have a smoking habit for about three and a half years, smoking approximately one pack every three to four days.)

With these symptoms, it is advisable to visit an outpatient clinic for a proper examination to understand the underlying issue accurately.

More Info
Itchy sensations in the urogenital area can be concerning, especially for a teenager like yourself. Based on your description, it sounds like you are experiencing itching around the frenulum (the band of tissue on the underside of the penis) and some discomfort in the urethra. There are several potential causes for these symptoms, and understanding them can help you determine the best course of action.
Possible Causes
1. Infections: One of the most common causes of itching in the genital area is an infection. This could be a sexually transmitted infection (STI) such as chlamydia or gonorrhea, especially since you mentioned being sexually active. Other infections, such as a yeast infection or bacterial infection, can also cause itching and discomfort.
2. Irritation: Sometimes, irritation can occur due to various factors, including soaps, detergents, or personal hygiene products. If you've recently changed any products or if you're using scented products, this could lead to irritation.
3. Skin Conditions: Conditions such as eczema or psoriasis can affect the genital area, leading to itching and discomfort. These conditions can cause the skin to become dry, red, and flaky.
4. Phimosis: If you are uncircumcised, phimosis (a condition where the foreskin cannot be easily retracted) can lead to irritation and discomfort. This can cause the skin to become tight and may lead to itching.
5. Hygiene: Inadequate hygiene can also lead to itching. It's important to keep the area clean and dry to prevent irritation and infections.
Recommendations
1. Consult a Healthcare Provider: Given that your symptoms have persisted for a month and include discomfort during urination, it is crucial to consult a healthcare provider. They can perform a physical examination, take a medical history, and possibly run tests to determine the cause of your symptoms.
2. Avoid Irritants: In the meantime, try to avoid any potential irritants. Use mild, unscented soaps and avoid any products that could cause irritation.
3. Maintain Hygiene: Ensure that you are practicing good hygiene. Clean the area gently with warm water and avoid scrubbing, which can exacerbate irritation.
4. Monitor Symptoms: Keep track of any changes in your symptoms, including the appearance of any rashes, discharge, or changes in urination. This information will be helpful for your healthcare provider.
5. Avoid Sexual Activity: Until you have seen a healthcare provider, it may be wise to avoid sexual activity to prevent further irritation or potential transmission of an infection.
Conclusion
While it can be uncomfortable and concerning to experience itching in the urogenital area, many causes are treatable. It is essential to seek medical advice to get a proper diagnosis and treatment plan. Remember, your health is important, and addressing these symptoms early can lead to better outcomes. Don't hesitate to reach out to a healthcare professional who can provide you with the support and guidance you need.
