Acne scars on the hands?

I would like to ask the doctor! I used to frequently get pimples on my arms and would often squeeze them.
Now, I have small pockmarks all over my hands as a result! They are unsightly, and I would like to know if there are any methods to remove the scars on my hands.
The scars have not faded even after one or two years...
Please help me, doctor.

Hui, 20~29 year old female. Ask Date: 2009/04/05

Dr. Jiang Yujiao reply Dermatology

Hello: There are three possible conditions for acne scars on the arm: 1.
Depressed scars - can be treated with laser resurfacing.
2.
Raised scars, known as keloids - can be treated with topical or intralesional corticosteroids or cryotherapy.
3.
It may not be a medical scar but rather brown pigmentation - which will gradually fade over time and requires evaluation by a dermatologist.

When it comes to treating acne scars, particularly on the hands, there are several effective options available that can help improve the appearance of the skin. Acne scars can be frustrating, especially when they persist for years after the initial breakouts. Here’s a comprehensive guide on how to approach the treatment of these scars.

Understanding Acne Scars
Acne scars can be classified into two main types: atrophic scars, which are depressed and often appear as pits or indentations in the skin, and hypertrophic scars, which are raised and can be red or darker than the surrounding skin. The type of scar you have will influence the treatment options available.

Treatment Options
1. Topical Treatments:
- Retinoids: Products containing retinoids, such as tretinoin or adapalene, can help promote cell turnover and improve skin texture. These should be used cautiously, especially if your skin is sensitive or if you have recently used other strong treatments.

- Alpha Hydroxy Acids (AHAs): These are exfoliants that can help remove dead skin cells and improve skin texture. Glycolic acid is a common AHA used for this purpose.

- Beta Hydroxy Acids (BHAs): Salicylic acid can penetrate into the pores and help with both acne and the appearance of scars.

- Vitamin C: This antioxidant can help brighten the skin and improve the appearance of scars over time.

2. Chemical Peels:
- Chemical peels involve applying a solution to the skin that causes the top layer to peel off, revealing fresher skin underneath. This can help reduce the appearance of scars. Superficial peels can be done more frequently, while deeper peels may require more downtime.

3. Microneedling:
- This procedure involves using a device with fine needles to create micro-injuries in the skin, which stimulates collagen production. Microneedling can be effective for both atrophic and hypertrophic scars.

4. Laser Treatments:
- Laser therapy can target specific layers of the skin to improve texture and tone. Fractional lasers are particularly effective for acne scars, as they promote healing without damaging the entire surface of the skin.

5. Dermal Fillers:
- For atrophic scars, dermal fillers can be injected to raise the depressed areas of the skin, making them less noticeable. This is a temporary solution and may require repeat treatments.

6. Surgical Options:
- In some cases, surgical procedures such as subcision (where a needle is used to break up the fibrous tissue beneath the scar) can be performed to improve the appearance of scars.

General Skin Care Tips
- Sun Protection: Always use sunscreen on scarred areas, as UV exposure can darken scars and make them more noticeable.

- Moisturization: Keeping the skin hydrated can help improve its overall appearance and texture.

- Avoid Picking: As you mentioned, picking at acne can lead to scarring. It's essential to avoid this habit to prevent further damage to the skin.

When to See a Dermatologist
If over-the-counter treatments and home remedies do not yield satisfactory results, it may be time to consult a dermatologist. They can assess your skin and recommend a tailored treatment plan based on the type and severity of your scars.

Conclusion
Acne scars can be persistent, but with the right treatment approach, significant improvement is possible. Combining topical treatments with professional procedures often yields the best results. Remember to be patient, as scar treatment can take time, and results may vary based on individual skin types and conditions. Always consult with a healthcare professional before starting any new treatment to ensure it is appropriate for your skin type and condition.

Many acne lesions indicate poor facial metabolism. Using a gentle cleanser to wash your face, avoiding squeezing pimples, and applying glycolic acid gel before bed are more conservative approaches. Glycolic acid peels and blue light therapy are also effective treatments.
