Timing issues related to the condition assessment?
Dear Dr.
Xu,
Thank you for patiently answering so many questions.
I really appreciate it.
I have been following your instructions and taking the antibiotic Ciproxin along with HArnalidge to treat my chronic prostatitis.
The previous issue of urinary leakage has improved; the first two days of medication showed significant improvement, and I no longer experience frequent leakage, just occasionally (a drop every three or four hours).
However, after the third day, up to the fifth day, my condition has remained the same as on the third day, with occasional feelings of incomplete bladder emptying and no further improvement.
Is this normal, or does the physician need to reassess the diagnosis? During the course of the medication, is it okay for me to drink 2000cc of water daily? Will this dilute the effectiveness of the medication? Thank you.
Chen R, 40~49 year old female. Ask Date: 2009/04/19
Dr. Xu Weikai reply Urology
Hello, it should be a normal phenomenon.
After all, medications cannot provide a rapid and complete cure, so a long treatment duration is necessary.
During this time, the condition will gradually improve due to the treatment, but it cannot be expected to improve immediately.
Of course, if the overall health condition is poorer, there may also be a phenomenon of symptom recovery.
If there are any unclear points, feel free to ask again, and we can discuss and research further.
Sincerely,
Dr.

Chronic prostatitis is a complex condition that can significantly impact a patient's quality of life. Your experience with symptoms such as urinary incontinence, incomplete bladder emptying, and the sensation of burning during urination is common among those diagnosed with this condition. Let's address your questions and concerns regarding the treatment timeline and recovery from chronic prostatitis.
Understanding the Timeline of Treatment and Recovery
1. Duration of Treatment: Chronic prostatitis can be notoriously difficult to treat, and the duration of treatment varies widely among individuals. While some patients may experience relief within a few weeks, others may require several months of therapy. The fact that you have noticed some improvement with the use of ciprofloxacin and HarnaLidge is encouraging, but it is not uncommon for symptoms to fluctuate during the treatment process.
2. Normalcy of Symptoms: The stabilization of your symptoms after a few days of treatment is a typical response. It is not unusual for patients to experience a plateau in symptom improvement after initial progress. This could be due to several factors, including the nature of chronic prostatitis itself, which can have periods of exacerbation and remission. If you continue to experience symptoms like incomplete bladder emptying or occasional leakage, it may be worth discussing with your physician. They might consider adjusting your treatment plan or exploring additional diagnostic tests to rule out other underlying issues.
3. Hydration and Medication Efficacy: Drinking adequate fluids is essential for urinary health, especially when dealing with prostatitis. A daily intake of 2000cc (or about half a gallon) is generally considered appropriate for most adults, provided there are no contraindications. However, excessive fluid intake can sometimes lead to increased urinary frequency, which may be uncomfortable. It is unlikely that this level of hydration would dilute the effectiveness of your medications, but it is always best to follow your doctor's recommendations regarding fluid intake, especially if you have specific health concerns.
4. Follow-Up Care: Regular follow-up with your healthcare provider is crucial in managing chronic prostatitis. If your symptoms do not improve or worsen, it is essential to communicate this to your doctor. They may recommend further evaluation, which could include additional urine tests, imaging studies, or even a referral to a specialist in urology or pelvic pain management.
Additional Considerations
- Lifestyle Adjustments: Alongside medication, lifestyle changes can play a significant role in managing chronic prostatitis. These may include dietary modifications (reducing caffeine and spicy foods), regular exercise, and stress management techniques. Stress can exacerbate symptoms, so incorporating relaxation techniques such as yoga or mindfulness may be beneficial.
- Alternative Therapies: Some patients find relief through alternative therapies, such as pelvic floor physical therapy, acupuncture, or low-intensity extracorporeal shock wave therapy (Li-ESWT). These treatments may help alleviate pelvic pain and improve urinary function.
- Probiotics and Gut Health: If you have been on antibiotics for an extended period, consider discussing the use of probiotics with your healthcare provider. Antibiotics can disrupt the balance of gut bacteria, and probiotics may help restore this balance, potentially improving overall health and well-being.
- Long-Term Outlook: Chronic prostatitis can be a long-term condition, and while some patients achieve significant improvement, others may experience ongoing symptoms. It is essential to maintain open communication with your healthcare provider and to be proactive in managing your condition.
In summary, while your experience with chronic prostatitis can be frustrating, it is important to remain hopeful and engaged in your treatment plan. Regular follow-ups, lifestyle adjustments, and open communication with your healthcare provider will be key components in managing your symptoms effectively. If you have further concerns or if your symptoms persist, do not hesitate to seek additional medical advice.
