Sexual dysfunction
Hello, Director Lin.
First of all, I would like to thank you for your diagnosis and response.
I would like to ask what might be causing my situation.
On the day of my visit, I noticed that the clinic hours were almost over, so I couldn't ask more questions.
Therefore, I will describe my condition in detail here.
I have not experienced any trauma to my testicles, and my sexual activity frequency is normal without any harmful habits.
My daily routine is also normal.
Previously, I would occasionally experience some pain in my testicles, but it was not long-lasting.
However, recently, the pain has become very frequent, often lasting for a considerable time, and sometimes it radiates to my lower back and causes pain in my penis during erections.
I also experience pre-ejaculatory fluid.
Additionally, I would like to mention the issue of erectile dysfunction that I previously discussed with you, which has not improved at all.
It feels like a double blow.
I don't feel stressed, but I previously thought my glans was overly sensitive, leading to premature ejaculation, so I underwent a circumcision at your facility.
However, the premature ejaculation has not improved, even though I have been applying the ointment you prescribed.
The duration of effectiveness is still very short.
Regarding the erectile dysfunction I mentioned earlier, I am still unable to achieve an erection (I have also noticed that the frequency of morning erections is decreasing).
If I engage in sexual activity, my penis becomes soft after about one minute of intercourse.
After that, it takes about half an hour to achieve another erection, but the hardness is not very firm (and I did not ejaculate at that time).
Due to these sexual dysfunctions, I have also been taking traditional Chinese medicine for treatment, but there has been no improvement.
I would like to ask you, Director, why at such a young age do I have premature ejaculation and erectile dysfunction as if they are labels stuck to me? How should Western medicine treat such sexual dysfunction? These are my questions, and I kindly ask for your thoughtful response.
Thank you.

Hello Sir: Thank you for your inquiry.
I would like to respond to your message and provide the following suggestions: Epididymitis is primarily caused by lower urinary tract infections and does not necessarily require the presence of adverse habits.
Regarding the issues of erectile dysfunction and premature ejaculation, these are common concerns among the male population.
However, you have not been diagnosed with erectile dysfunction, and you do have some erectile function.
The issue of premature ejaculation can have psychological effects.
In addition to the treatments and advice previously provided to you, recent medical reports have confirmed that medications such as Viagra (sildenafil) or Cialis (tadalafil) are effective in enhancing erectile function and also provide meaningful prolongation of ejaculation time for premature ejaculation.
If you are considering treatment options, this is a good approach without incurring excessive costs.

More Info
Erectile dysfunction (ED) is a complex condition that can arise from various physical, psychological, and lifestyle factors. Based on your description, it seems you are experiencing a combination of symptoms that may be interrelated. Let's break down the potential causes and treatment options for your situation.
Possible Causes of Erectile Dysfunction
1. Physical Factors:
- Hormonal Imbalances: Low testosterone levels can significantly impact libido and erectile function. Hormonal evaluations can help determine if this is a contributing factor.
- Vascular Issues: Conditions affecting blood flow, such as atherosclerosis or hypertension, can impede the ability to achieve or maintain an erection. A thorough cardiovascular assessment may be warranted.
- Neurological Conditions: Nerve damage from conditions like diabetes or multiple sclerosis can lead to ED. If you have any underlying neurological issues, they should be evaluated.
- Pelvic Injuries: Trauma to the pelvic area, including the penis and testicles, can lead to erectile dysfunction. Given your mention of pain in the testicles and lower back, this could be a significant factor.
2. Psychological Factors:
- Anxiety and Stress: Performance anxiety, especially related to previous experiences of ED or premature ejaculation, can create a cycle of anxiety that exacerbates the problem.
- Depression: Mental health issues can significantly impact sexual function. If you are feeling down or anxious about your sexual performance, it may be worth discussing with a mental health professional.
3. Lifestyle Factors:
- Substance Use: Alcohol, tobacco, and recreational drugs can negatively affect erectile function. If you consume these substances, consider reducing or eliminating them.
- Diet and Exercise: A poor diet and lack of physical activity can contribute to obesity and cardiovascular issues, which are risk factors for ED. A balanced diet and regular exercise can improve overall health and potentially enhance erectile function.
Treatment Options
1. Medical Treatments:
- Phosphodiesterase Type 5 Inhibitors (PDE5i): Medications like sildenafil (Viagra), tadalafil (Cialis), or vardenafil (Levitra) are commonly prescribed for ED. They work by increasing blood flow to the penis and can be effective for many men.
- Hormone Therapy: If low testosterone is identified, hormone replacement therapy may be an option.
- Injection Therapy: For some men, self-injecting medication directly into the penis can help achieve an erection.
2. Psychological Counseling:
- If psychological factors are contributing to your ED, therapy with a psychologist or sex therapist can be beneficial. Cognitive-behavioral therapy (CBT) can help address anxiety and improve sexual performance.
3. Lifestyle Modifications:
- Improving diet, increasing physical activity, and reducing stress can have a positive impact on erectile function. Consider consulting with a nutritionist or personal trainer for tailored advice.
4. Surgical Options:
- In cases where other treatments fail, surgical options such as penile implants may be considered. This is typically a last resort after other treatments have been tried.
Conclusion
Your symptoms of erectile dysfunction, along with the associated pain in the testicles and lower back, warrant a comprehensive evaluation by a urologist. It is essential to address both the physical and psychological aspects of your condition. Open communication with your healthcare provider about your symptoms, concerns, and treatment preferences is crucial for finding an effective solution. Remember, you are not alone in this, and many men experience similar issues. With the right approach, improvement is possible.
